三种腹膜透析装置使用的效果评价

摘　　要：目的　对海南省 3种腹膜透析装置的使用效果进行评价 ,为临床护理及肾功能衰竭患者提供良好的治疗 ,降低腹膜炎发生率 ,提高生存率。　方法　 1981～ 2 0 0 2年省工人医院 189例腹膜透析患者 ,按透析装置分为 :Ⅰ组 (自接Y型管道组 ) ,Ⅱ组 (O型管道组 ) ,Ⅲ组 (双联管道组 ) ,从其操作难易程度、腹膜炎发生率、透析存活时间等方面进行效果评价。　结果　双联管道组操作易于掌握 ,培训时间短 ,腹膜炎的发生率明显低于自接Y型管道组 (P <0 0 5 ) ,稍低于O型管道组 (P <0 0 5 ) ;透析存活时间明显高于自接Y型管道组 (P <0 0 5 ) ,略高于O型管道组 (P <0 0 5 )。　结论　使用双联管道装置开展腹膜透析 ,可明显降低腹膜炎的发生率 ,其操作简单 ,易于掌握 ,培训时间短 ,是热带地区开展腹膜透析特别是家庭腹膜透析的良好装置。Objective To evaluate and compare the effects among three peritoneal dialysis sets in the tropical districts of Hainan, for providing better services for the kidney failure patients, and reducing the incidence of peritonitis and increase the survival rate. Methods 189 patients had undergone peritoneal dialysis in the hospital during the past 22 years,and were randomly divided into three groups in accordance with time and devices used,namely Group I(self-employed type Y links),Group Ⅱ(Type O links)and Group Ⅲ(double links),the results of manipulation, the incidence of peritonitis and length of dialysis survival and so on were analysed.Results Devices in GroupⅢ are more easily manipulated and need shorter training periods. The incidence of peritonitis in Groop Ⅲ is significantly lower than in Group Ⅰ (P<0.05)and Group Ⅱ(P<0.05).The length of survival in GroupⅢ is greatly longer than that in Group Ⅰ,but only slightly higher than that in Group Ⅱ. Conclusion The incidence of peritonitis can be reduced apparently with the double links dialysis sets, which are much more easily operated and require shorter training periods. It is an ideal device of peritoneal dialysis for family-use.
