胸主动脉夹层腔内隔绝术后切口区并发症的处理

摘　　要：目的　总结胸主动脉夹层术后切口区并发症的原因、处理及预防方法。方法　回顾性分析 1998年 9月至 2 0 0 1年 12月间胸主动脉夹层腔内隔绝术 116例 ,其中选择腹股沟斜切口 87例 ,下腹部腹膜外切口 2 5例 ,由腹股沟斜切口延长为下腹部斜切口 4例。结果　术后发生切口感染 3例 ,淋巴瘘 10例 ,切口血肿 7例 ,右侧下肢动脉血栓形成 1例 ,股外侧皮肤的麻木感 6例。经过抗感染、引流、放疗和取栓等措施 ,切口均愈合。结论　要认识到腔内隔绝术切口区特有的并发症 。ObjectiveTo investigate the cause and cure of groin inc ision com plications after endovascular graft exclusion (EVGE) for thoracic aortic dissect io n(TAD).MethodsA retrospective analysis was made in 116 cases of TAD from September 1998 to December 2001 Of them, 87 received inguinal oblique incisions, 25 hypogastric retroperitoneal incisio ns and 4 retroperitoneal incisions prolonged by inguinal oblique incisions.ResultsPostoperative infection of incisions oc curred in 3 cases,the lymphatic fistula in 10 cases, the incision hematoma in 7,the arterial thr ombosis of right low limb in 1 and skin numbness of lateral thigh in 6. By measu res of antibiotics,drainage,radiation therapy and embolectomy,all incisions healed.ConclusionIt is significant to stress the special incision complications of EVGE and select the proper treatment.
