摘　　要：目的 :分析我院从 1 997年 1 1月至 2 0 0 1年 1 1月采用Ikeda钉合法治疗的 6 0例先天性巨结肠患儿的近远期疗效。方法 :患先天性巨结肠的 6 0名儿童 ,年龄 5月～ 5岁 ,普通型 4 4例 ,长段型1 0例 ,短段型 6例 ,均采用Ikeda钉合法治疗 ,术后随诊 2～ 6年。随诊内容包括近期术后并发症 ,远期肛门排便功能测定及生活质量情况。结果 :本组病例中无围手术期死亡的。近期并发症 :4例患儿出现小肠结肠炎 ,约占 6 .7% ;7例患儿出现少量血便 ,约占 1 1 .7% ;便秘 1 2例 ,约占 2 0 %。远期随诊患儿无肛门排便失禁及长期污粪者 ,肛门功能优良率达到 96 .7% ,生活质量优良。结论 :采用Ikeda钉合法治疗先天性巨结肠虽受年龄限制 ,但该术式的近期并发症少 ,术后恢复快 ,远期疗效优良 ,生活质量高。Objective: To analyze the long-term curative effect about the Ikeda’s anastomat operation procedure for 60 Hirschsprung disease children followed up.Method:60 cases of Hirschsprung disease ,whose age ranged from 5 months to 5 years old, including 44 common cases,10 long seyment cases and 6 short seyment cases, were all treated by Ikeda's anastomosis using nails. The followed-up term is from 2 years to 6 years long, that was 4.8 years by average. And the contents were the postoperative complications estimate of anus defecation and patients quality of lives.Result: The mortality rate was down to zero in the perioperative period.The postoperative complication included 4 cases of enterocolitis, about 6.7%, 12 cases of small quantity blood occurred in stool, 11.7%,and 12 cases of condtipation, 11.7% of the total. No case had the incontinence of anus defecation and long-term dirty stool. The anal function of the cases were fine in 96.7%(LiZheng.Score).The quality of life were fine,too. Conclusion:Though of the limitation of age,the procedure of Ikeda's anastomat operation with nails had few postoperative complications.After operative patients got fast postoperative recovery, resistant effectand high quality of lives.
