复方氯芬待因片药物依赖性流行病学调查

摘　　要：目的 :调查评价复方氯芬待因片的药物依赖性。方法 :采用“回顾性调查”和“集中监测”相结合的方法 ,对北京、上海、天津三地区多家医疗单位就诊患者中使用复方氯芬得因片的情况进行调查。结果 :共调查 5 86例符合条件的复方氯芬得因片使用者 ,平均连续使用时间为 ( 17± 8)d ,绝大多数患者用于治疗各种原因引起的疼痛。VAS测量结果表明 ,复方氯芬待因片可以有效缓解中等程度 (VAS值为 5 .4 2cm)的疼痛。VAS测量复方氯芬待因片欣快效应的结果显示 ,致欣快效应VAS值几乎为 0 ;除个别患者在停用复方氯芬待因片后出现轻度 (极个别出现中等程度 )戒断症状外 ,绝大多数患者在停药后无戒断症状 ,各项症状OWS平均分值介于 0 .0 1～ 0 .0 5之间。初始时和最多时复方氯芬待因片平均使用日剂量分别为 3.3和 4 .6片 ,两者比较差异具有显著性 (t =18.4 80 ,P =0 .0 0 0 )。结论 :医疗用途常规剂量使用复方氯芬待因片药物依赖性潜力很低 。Objective: To survey and assess the drug dependence of diclofenac sodium and codeine phosphate tablet (DSC). Method: Subjects used DSC for 7 days at least were surveyed by retrospective survey with intensive hospital based survey. Physical dependence and psychic dependence were assessed by opiate withdrawal scale (OWS) and visual analogue scale (VAS), respectively. Result: A total 586 subjects from Beijing, Shanghai and Tianjin participated in this study. The mean time of used DSC was (17±8) days. The main purpose of used DSC was to treat pains resulted from the various reasons. The results suggested that DSC could relief moderate pain. Except a few patients had slight and moderate withdrawal syndrome, most patients did not occur the withdrawal syndrome after discontinuantion DSC. The arerage score of OWS was between 0.00 0.05 . The psychic dependence assessed by VAS was low. The initial and later doses of DSC use were 3.3 and 4.6 tablets per day, respectively. There was a significant difference between initial dose and later dose ( t = 18.480 , P = 0.000 ).Conclusion: The drug dependence of DSC is low for the use of medical purpose. The tolerance of DSC may be occured after the use of long time.
