摘  要:目的:应用高频超声显像技术观察临床患者颈动脉和股动脉内膜-中层厚度(IMT)的自然演变。方法:在28例患者中,应用高频超声技术测量颈动脉和股动脉的分叉前2cm、分叉部及分叉后2 cm范围内的动脉后壁正常部位IMT值(IMTA),最大IMT值(IMTB),四条动脉最大IMT平均值(IMTC),四条动脉中单个最大IMT(IMTD),同时测量管壁的僵硬度(β值),3年后复查。结果:入选时及3年后颈动脉和股动脉的IMTA、与β值差异均无统计学意义。右颈动脉斑块部位IMTB显著增加(P<0.05),斑块部位IMTB的变化值显著大于正常部位IMTA的变化值。颈动脉IMTA和IMTB的变化值与右股动脉均值差异无统计学意义(P>0.05)。IMTA的平均年增长速度为0.023 mm,IMTC和IMTD的平均年增长速度分别为0.075 mm、0.14 mm。结论:应用高频超声技术能够可靠的观察颈动脉和股动脉内膜-中层厚度(IMT)的消长改变。Objective: To investigate the natural progression process of intima-media thickness (IMT)in carotid and femoral arteries with high-frequency ultrasound imaging. Methods: Both sides of carotid and femoral arteries were detected with a 4-10 MHz transducer and the intima-media thickness was measured. IMT of normal segment in each artery was defined as IMTA , the maximal IMT in each artery as IMTs , the mean IMTb of four arteries as IMTc and the maximal IMT of 4 arteries as IMTD, Follow-up was performed in 3 years by ultrasound images. Results: After 3 years, IMTA and rigidity (β) showed no significant differences from those of the baseline. The thickness progression of IMTb was larger than that of IMTA (P≤0. 05). The difference of IMTA or IMTb between carotid arteries and femoral arteries was not significant (P>0. 05) , The mean progression rates per years of IMTA, IMTc and IMTD were 0. 023 mm, 0. 075 mm and 0. 14 mm respectively. Conclusions: The natural progression of the IMT in atherosclerotic segments was faster than that in normal ones in the same patients, and the progressions showed no significant difference between carotid and femoral arteries. The high frequency ultrasound technique provides a reliable routine method for monitoring atherosclerotic progression in carotid and femoral arteries.
