孤立性右心室发育不全的综合影像学诊断(附2例报告)

摘　　要：目的 :探讨影像技术对孤立性右心室发育不全的诊断价值。方法 :分析 2例孤立性右心室发育不全的影像学表现 ,结合文献评价各种影像技术对本病的诊断和鉴别诊断作用。结果 :X线平片显示肺血减少 ,右心房、左心室增大 ;心血管造影、心脏超声和 MRI显示右心室腔明显缩小、右心房、左心室增大、房间隔缺损以及房水平左向右分流。结论 :心血管造影、心脏超声和MRI均能显示本病的病理解剖和血流动力学改变。在本病的诊断和鉴别诊断中 ,综合应用非创伤性、无辐射损害的影像技术有可能取代创伤性的心血管造影检查。Objective:To investigate the value of diagnostic imaging for isolated right ventricular hypoplasia.Methods:2 patients with isolated right ventricular hypoplasia were studied with chest X-ray, ultrasonic cardiography, cardioangiography and MRI. The imaging findings was evaluated in isolated right ventricular hypoplasia.Result:Chest X-ray showed pulmonary oligemia, right atrial and left ventricular enlargement. The diminution of the right ventricle, enlargement of right atrial and left ventricular were showed on angiocardiography, ultrasonic cardiography and MRI. An atrial septal defect with right to left shunting at the atrial lever was demonstrated.Conclusion:Angiocardiography, ultrasonic cardiography and MRI can show morphologic and hemodynamic features of isolated right ventricular hypoplasia. The combined non-invasive imaging techniques should replace invasive angiocardiography for diagnosis of isolated right ventricular hypoplasia.
