摘  要:目的 探讨米索前列醇 (简称米索 )用于足月妊娠引产对妇女血浆内皮素 -1(ET-1)和一氧化氮 (NO)水平的影响。 方法 符合试验条件的足月妊娠初产妇 10 6例随机分组 ,56例以米索 50 μg和 50例以米索 2 5μg放置于阴道后穹窿 ,每 3 h1次 ,直至临产停药 ,1日用药以 4次为限。选择同期住院 ,自然临产的初产妇 56例为对照组。用放射免疫法和硝酸还原酶法测定 3组中部分孕产妇产程不同时期血浆中 ET-1和 NO水平。 结果  (1)用药至规律宫缩时间米索 50 μg组为 (3 .99± 2 .2 5) h明显短于 2 5μg组的 (5.12± 2 .3 8) h,差异有显著性 (P<0 .0 5) ;第一产程及总产程时间比较 ,米索 50μg组短于 2 5μg组和对照组 ,差异有显著性 (P<0 .0 5)。 (2 ) 3组产妇用药至临产后血浆内皮素水平逐渐升高 ,至第二产程达高峰 ,胎盘娩出后 0 .5h迅速下降至临产前水平 ,血浆 NO水平即随着产程的进展逐渐下降 ,至第二产程时降至最低水平。 (3 )对照组临产前妇女血浆ET-1水平明显高于米索 50 μg及 2 5μg组用药前 ,而 NO水平则明显低于上述 2组用药前 ,差异有显著性 (P<0 .0 5) ,3组产程的其它不同时期 ET-1和 NO水平比较 ,差异无显著性 (P>0 .0 5)。 结论 孕妇血浆 ET-1水平升高和 NO水平降低与分娩发动密切相关。Objective To explore the effects of the levels of endothelin 1(ET 1) and nitric oxide (NO) in misoprostol in induced labor for full term pregnancy. Methods The total 106 primiparas at term pregnancy with inclusion criteria were divided randomly into two groups.The 56 subjects in group A received 50 μg misoprostol,for every 3 hours for vaginally,and the 50 subjects in group B received 25 μg misoprostol for every 3 hours for vaginally.All subjects received drug constantly until the onset of labor and the highest frequency was 4 per day.A total 56 primiparas being in hospital in the same period were treated as the control group D.The vein blood of partial women of the three groups was taken in different labor stage.ET 1 concentrations were measured with RIA,and NO levels were determined with nitrite reductase assay. Results (1)Between the two test groups,the internal from administration to the onset of labor and inducation in group A was significantly shorter than that in group B statistically (P<0 05).(2)For all the three groups,ET 1 levels increased gradually with the development of labor,reached the highest level in the second stage,and decreased to the level before labor onset half an hour after the delivery of placenta.The change of NO levels showed gradual decreasing trend with the development of labor,and the concentration reached the lowest level after placenta delivery.(3)ET 1 levels before labor onset in the control group were higher statisticlly than that of preadministration in the test groups(P<0 05),but the NO levels were opposite to it(P<0 05).In the postpartum stage,there was no statistical difference in the same period among the three groups on ET 1 and NO levels. Conclusion The increase of ET 1 level and the decrease of No level in the plasma of pregnancy women are associated with labor onset.Misoprostol can affect the labor development by altering ET 1 and NO levels.The changing scale depends on the administration dosege,and the changing rule is the same as that in physical de
