摘　　要：目的 :了解在神经外科病区的病人中鲍曼不动杆菌感染的耐药情况。方法 :对近 3年我科从住院病人痰液中分离的 39株鲍曼不动杆菌的药敏情况作回顾性分析。药物敏感试验采用的是法国梅里埃API系统 ,纸片扩散法。参照标准为美国临床实验标准委员会 1999年的判断标准。结果 :共分离不动杆菌 4 4株 ,其中鲍曼不动杆菌39株 ,占 88 6 %。经药敏试验测定 ,分离出的鲍曼不动杆菌对碳青霉烯类抗生素的亚胺培南高度敏感 ,无耐药 ;对第三代头孢菌素类抗生素中的头孢哌酮 ,头孢哌酮 /舒巴坦 ,头孢曲松 ,头孢他啶的耐药率分别为 5 1 5 %、13 3%、6 1 1%、2 7 1% ;而对第四代头孢菌素头孢吡肟的耐药率达 2 4 % ;在氨基糖苷类中 ,分离菌株对阿米卡星耐药率为2 1 1% ,对庆大霉素为 35 %。结论 :鲍曼不动杆菌是一种多重耐药菌 ,体外药敏试验显示 :亚胺培南、头孢他啶、头孢吡肟、头孢哌酮 /舒巴坦。Objective:To find out the antibiotic resistance of Acinetobacter Baumannii in inpatient area of a neurosurgical ward. Methods: A. Baumannii was indentified by BioMerieux API system and drug susecptible tests were performed using disk diffusion methods, according to the standard of the guidelines of NCCLS(1999). 39 strains of A. Baumannii extracted from patients in our neurosurgical ward in recent 3 years were retrospectively analysed. Results:39 strains of A. Baumannii were extracted from the total 44 strains of Acinetobacter, and the rate was 88 6%. Imipenem(carbapenaems antibiotics) was most susecptible and not resistant. The resistant rate of Cefoperazone, Cefoperazone/Sulbactam, Ceftazidime and Ceftriaxone(Ⅲ cephalosporins antibiotics) was 51 5%,13 3%, 61 1% and 27 1%, respectively. And the Cefepime(Ⅳcephalosporins antibiotics) resistant rate was 24%, respectively. The resistant rate of Amikacin and Gentarmicin(aminoglycosides antibiotics) was 21 1% and 35%. Conclusions: A. Baumannii is a kind of multiple drug resistant microbe. It was observed from in vitro antimicrobial activities that Imipenem, Cefepime, Cefoperazone/Sulbactam and Amikacin could be used to treat the A. Baumannii infection. [
