摘　　要：[背景 ]以无痛性肉眼血尿为主的腹主动脉夹层动脉瘤病例罕见 ,易误诊 .[病例报告 ]患者年龄为 6 1岁 ,男性 ,以无痛性肉眼血尿伴血块为主要症状 ,做主动脉造影得以确诊 .因病变累及范围广 ,且累及肾动脉 ,故放弃手术治疗 ,只行保守治疗 .[讨论 ]腹主动脉夹层动脉瘤累及肾动脉时 ,也能引起无痛性肉眼血尿 ,应与膀胱肿瘤等加以鉴别 .BACKGROUNDThe abdominal aortic dissecting aneurysm presenting with painless gross hematuria as the main symptom is rare in the clinic practice, and could easily be misdiagnosed.CASE REPORTA 61 years old male patient complained of painless gross hematuria and clot in the urine without any other uncomfortable feeling. Angiography revealed abdominal aortic dissecting aneurysm with involvement of left renal artery, which was considered to be contraindication for surgical intervention. The situation of the patient was stable under conservative treatment.DISCUSSIONThe abdominal aortic dissecting aneurysm could lead to painless gross hematuria which mimic urinary bladder tumor at presentation, therefore attention should be paid for differential diagnosis in daily clinical practice.
