CDE在诊治PTCA术后股动脉假性动脉瘤中的应用价值

摘　　要：【目的】探讨彩色多普勒能量图 (CDE)对经皮穿刺腔内冠状动脉成形术 (PTCA)术后股动脉假性动脉瘤 (PA)的诊断和治疗价值。【方法】17例PTCA术后穿刺部位发现肿块和 (或 )听诊有血管杂音的患者 ,采用CDE检查股动脉及其周围组织 ;在CDE引导下对股动脉PA进行单纯性压迫和 /或瘤腔内注入巴曲酶加压迫治疗。【结果】17例PA患者CDE全部明确诊断 ,与临床和MRI符合率 10 0 % (17/ 17)。CDE引导下 12例PA经单纯压迫治愈 ,4例PA经瘤腔内注入巴曲酶加压迫治愈 ,其治愈率为 94 .1% (16 / 17) ;1例PA经CDE引导压迫治疗失败而行外科手术修补。【结论】CDE对PTCA术后股动脉PA有较高的诊断价值 ,且在引导压迫、瘤腔内注药治疗PA方面简单、安全、有效。To evaluate the application of color Doppler energy imaging(CDEI) in the diagnosis and treatment of femoral arterial pseudoaneurysms(PA) following percutaneous transluminal coronary angioplasty (PTCA).Femoral arteries and periarterial tissues in 17 patients with a mass and/or vascular murmur at the puncture site after PTCA were examined by CDEI. PAs were treated under CDEI guidance through simple compression and/or direct injection of batroxobin into their lumens plus compression.PAs in 17 cases were diagnosed definitely by CDEI, and the rate of conformity with their clinical finding and MRI was 100%(17/17).Twelve cases of them were healed with simple compression under CDEI guidance, while 4 cases recovered from percutaneous injection of batroxobin into the PA lumens plus compression ; the success rate was 94.1%(16/17). Surgical operation was done in one case after failure of treatment by compression under CDEI guidance.[Conclusion]CDEI exhibits high diagnostic significance for PA following PTCA. The measure of compression under CDEI guidance and batroxobin injection into PA lumens are simple, safe and effective.
