皮肌炎伴发恶性肿瘤82例临床分析

摘　　要：目的　了解皮肌炎伴恶发性肿瘤患者的临床特征。方法　对近 2 0年住院的 82例皮肌炎伴发恶性肿瘤患者进行临床分析。结果　 82例均伴发恶性肿瘤的患者 ,以 3 0岁～年龄段的发生率较高 ,上皮组织来源的肿瘤占绝大多数 ,其中鼻咽癌最常见。皮肌炎多出现在恶性肿瘤发生之前 ,时间为 1～ 6个月 ,但也可两者同时发生或恶性肿瘤先于皮肌炎发生。其中 2 6例死亡 ,5 6例患者经手术治疗肿瘤后 ,皮肌炎症状明显好转甚至完全消失。结论　皮肌炎与恶性肿瘤关系密切 ,对于 3 0岁～年龄段的皮肌炎患者 ,应该尽量寻找恶性肿瘤并进行治疗。Objective To study the clinical features of dermatomyositis associated with malignancy. Methods The clinical manifestations of 82 hospitalized patients with dermatomyositis associated wtih malignancy in recent 20 years were analyzed. Results All the 82 patients with dermatomyositis were associated with malignancies, especially in patients aged over 30 years. Most malignancies were found in the epithelial tissues and the pharynx cancer was commonly seen. The occurrence of most dermatomyositis preceded malignancy by 1 to 6 months, but it also occurred simultaneously or dermatomyositis following malignancy. Among these patients, 26 of them died of malignancy. With steroid and operation therapy treatment, the other 56 patients survived. Conclusion Malignancy is frequently associated with dermatomyositis. For the patients with dermatomyositis, especially those over 30 years old. the careful examination of malignancy and treatment would be very important.
