摘 要:目的 观察严重烧伤病人心肌酶谱的变化及 1,6 二磷酸果糖 ( 1 6 FDP)对它的影响。方法 采用随机双盲对照法 ,将烧伤面积在 3 0 %~ 60 %,Ⅲ度在 10 %~ 3 0 %的 3 8例病人及 2 0例健康志愿者 ,分为三组即 1,6 二磷酸果糖治疗组 (A组 ,n =2 1) ,安慰剂对照组 (B组 ,n =17) ,C组为健康志愿者 (n =2 0 )。A组静滴FDP 5 g( 5 0ml) 1次 /d ,B组静滴相同容量的的生理盐水 ,疗程为 10d。检测A ,B两组入院即时、伤后(PBD)第 1,2 ,5 ,10 ,14天烧伤病人的血清心肌酶谱 (LDH ,CK ,CK MB ,GOT)的变化和入院即时、伤后 (PBD)第 1,2 ,7,14 ,2 1天的心电图情况。结果 A ,B两组各时相点的LDH ,CK ,CK MB ,GOT值与C组比较明显升高 (P <0 0 5 ) ,且A组第 5 ,10 ,14天均低于B组 (P <0 0 5 ) ,A组伤后第 10天心肌酶的同功酶 (CK MB)已接近正常水平 ,B组第 14天时才接近正常。结论 烧伤后尽早应用心肌保护药有助于心肌功能的恢复 ,减轻烧伤后心肌损害程度 ,可作为严重烧伤病人的常规治疗措施之一。Objective To investigate the protective effect of fructose 1 6diphosphate(1 6 FDP)on myocardial injury with severely burned patients.Methods 58 patients were dividid into three groups.The patients of group one(B group, n =17) received fluid resuscilation only.The patients of group two(A group, n =21) were treated with resuscilation plus 1 6 FDP.Twenty healthy volunteers were enrolled in the control group(C group, n =20).Lactate dehydrogenase(LDH),creatine phosphokinease(CK),creatine phosphokinease isoenzyme(CK MB),glutamin oxamide transaminase(GOT) in plasma were tested in patients of all grous immediately after hospitalization and 1,2,5,10,14 postburn day(PBD).Electro cardiogram(ECG) were employed to monitor cardiac function in patients of all groups immediately after hospitliztion and 1,2,7,14,21 postburn day(PBD).Results (1)The serum LDH,CK,CK MB and GOT levels all time points in patients in A and B groups were evidently higher than those in C grorp ( P <0.05).(2)The rate of abnormal findings of ST T wave with ECG in B group was higher than that of A group( P <0.05).Conclusion Our data demonstrate that the admiuistration of 1 6 FDP in addition to fluid resuscitation is beneficial to burn injury,especiaily to myocardial injury,and it provides a promising approath for the fature of burns,as ponsibble as early after burn.
