电视腹腔镜下侧脑室肝膈间隙分流术治疗脑积水的对比研究

摘　　要：目的　总结电视腹腔镜下侧脑室 -肝膈间隙分流术与常规侧脑室—腹腔分流术的疗效对比。方法　回顾性分析 1996年 2月～ 2 0 0 2年 6月收治的 70例脑积水患者 ,其中腹腔镜治疗组 39例 ,常规手术组 31例。结果　腹腔镜组分流管腹腔端梗阻发生率为 12 .8% ,常规手术组梗阻发生率为 32 .3% (P <0 .0 5 )。术后感染率分别为 2 6 %和 3.2 % ,χ2 检验无显著差异。随访调查karnofsky评分 ,腹腔镜治疗组平均值 (6 9.3±11.5 ) ,常规手术组 (5 7.4± 10 .3) ,两组间无显著性差异 (P <0 .0 5 )。结论　电视腹腔镜下侧脑室肝膈间隙分流术治疗脑积水并发症少 ,是一种安全有效、值得推广的治疗方法。Objective: To present the therapeutic effect of hydrocephalus with TV -laparoscope in contrast with routine ventricular- hepato- diaphramatic shut. Methods:Seventy cases with hydrocephalus treated from February 1996 to June 2002 were retrospectively reviewed ,laparoscope operation group 39 cases,31 cases in routine operation group.Results:The obstruction rate of shut tube was 12.8% in laparoscope group and routine group was 32.3% ( P < 0.05 ) . The infection rate was respectively 2.6% and 3.2% ,χ 2 test had no signifigant difference. The Karnofsky score mean was (69.3±11.5) and (57.4±10.3) ( P < 0.05 ).Conclusions:The ventricular - hepato - diaphramatic shut with TV -laparoscope has rare complications and is a safe,effective,method used widely for hydrocephalus.
