超声刀在腹腔镜下子宫切除术中的应用分析

摘　　要：目的　通过腹腔镜下应用超声刀行子宫切除术 ,与未使用超声刀的对照 ,分析其实用价值。方法　研究组使用超声刀对 389例子宫良性病变需切除子宫者行CISH ,12 0例行CAVF ,4例子宫内膜癌行CAVF加淋巴清扫。对照组使用单、双极电凝对 4 0 0例子宫良性病变患者行CISH ,10 0例行CAVF ,分别对手术时间、术中出血量、手术前后最高体温、最高白细胞计数及血红蛋白浓度的均数进行比较。结果　全部病例手术经过顺利 ,无并发症发生。两组手术时间、术中出血量、手术前后最高体温、最高白细胞计数及血红蛋白浓度的均数比较差异有显著性。术后进行 1～ 6个月的随访 ,均无慢性腹痛及胃肠功能紊乱等现象出现。所有病例的宫颈外鞘恢复近似正常。盆腔组织质地柔软 ,性生活满意。结论　超声刀具有组织损伤小、少烟、少结痂、一剪多用等优势 ,是一种实用性强、值得普遍推广的腹腔镜手术刀。Objective To analyzes the clinical value of the ultrasound knife in the laparoscopy hysterectomy.Methods The researched group included 389 cases with the uterine benign diseases who had the hysterectomy, whose procedures were performed by CISH, and 120 cases with the uterine malignant diseases who were operated by means of CAVF plus the pelvic lymph oratory. Meanwhile, the controlled group included 400 cases of the volunteer receiving CISH and 100 patients of the CAVF, ultimately, mean of the observable parameters (e.g., intra-operative blood loss, operative room time, preoperative body temperature, leukocyte count, and hemoglobin concentration) of all subgroups’ patients were statistically comprised each other. Results Comparing the research group Vs control group, it showed a remarkable difference in mean of the observable parameters of intra-operative blood loss, operative room time, preoperative body temperature, leukocyte count, and hemoglobin concentration. Conclusions The ultrasound knife is a operative reality instrument which deserves us to the widespread popularization. It is a multi-use knife which possesses a variety of advantages, including less smoke, less scab as well as less fenestration damage.
