摘  要:目的 探索一种预防全喉切除后气管造口狭窄的新方法。方法 分别用镍钛记忆合金气管造口支架埋置法和单纯气管造口法在全喉切除时行Ⅰ期造口成型 ,6个月后比较它们的造口平均直径及呼吸、排痰功能的顺畅程度。结果 行支架造口成型 16例 ,单纯造口 2 5例 ,前者造口基本为圆形 ,形状稳定 ,平均为直径为 ( 13 .2± 3 .1)mm ,能满足呼吸及排痰需要 ;后者的气管造口均逐渐出现了造口挛缩 ,16例须长期放置气管导管才能维持正常呼吸。结论 全喉切除后镍钛记忆合金支架埋置能预防气管造口狭窄。Objective To explore a new method for the prevention of tracheostomal stenosis (TSS) following total laryngectomy. Methods Tracheostomy with and without nickel titanium shape memory alloy stent was performed following total laryngectomy. The mean diameter of the tracheal opening and functions of breath and expectoration were compared 6 months later. Results Steady round opening with an average diameter of (13.2±3.1) mm which could meet the requirements of breath and expectoration was found in 16 cases undergoing tracheostomy using the stent. Gradually contracted tracheal opening was found in 25 cases undergoing simple tracheostomy, and normal breath could be maintained by means of long term placement of endotracheal tube. Conclusion Placement of nickel titanium shape memory alloy stent following total laryngectomy can prevent tracheostomal stenosis.
