正常人左室心肌节段局部三维容量和收缩功能的定量研究

摘　　要：目的　应用动态三维超声心动图技术评价正常人的左室局部三维容量和局部收缩功能。方法　 4 1例健康查体人员 ,男性 2 4例 ,女性 17例 ,年龄 2 0～ 70岁 ,平均 (4 2± 12 )岁。采用 Agilent Sonos 5 5 0 0型彩色多普勒超声心动仪进行心脏二维图像的采集。采用 Tom Tec Gmb H型三维图像处理系统 ,以表面提取法重组心内膜边缘轮廓三维图像 ,自动计算每一节段的容量及收缩功能。结果　各节段局部舒张末容量测值从心尖到心底局部舒张末容量呈递增趋势 ;局部心搏量从后壁、下壁、侧壁、前间隔、前壁依次递增 ,在同一节段局部心搏量基本上是从心尖至心底依次递增 ;局部射血分数按前间隔、前壁、侧壁、后壁、下壁的顺序递减 ;各节段的局部 -整体射血分数按前间隔、前壁、侧壁、后壁、下壁的顺序递减。结论　应用动态三维超声心动图对局部心功能研究表明左室各节段收缩功能的三维分布并非均匀一致 ,收缩功能从后壁、下壁、侧壁、前间隔到前壁依次递增。Objective:To explore the uniformity/nonuniformity of left ventricular systolic function and establish the normal value of regional stroke volume,regional ejection fraction and other regional function parameters of left ventricle in normal population.Methods:The study population consisted of 41 normal subjects,including 24 men and 17 women.Two-dimensional image data were acquired using Agilent Sonos 5500 echocardiography imaging system.Three-dimensional reconstruction was performed using TomTec Gmb H three-dimensional imaging system equipped with 4D-LV analysis Ver.1.0 software.Results:The regional stroke volume increased in the order of posterior,lateral,anteroseptal and anterior wall successively,and there is an increasing trend in regional stroke volume from apex to base. The regional ejection fraction was nonuniform and decreased in the order of anteroseptal,anterior,lateral,posterior,and inferior wall. Likely,the regional-global ejection fraction was different among the 16 segments and decreased in the order of anteroseptal,anterior,lateral,posterior and inferior wall.Conclusions:The systolic function of left ventricle is asymmetric and becomes increasingly in the order of posterior,inferior,lateral,anteroseptal and anterior wall.
