三维彩色多普勒超声心动图测量心搏出量的临床研究

摘　　要：目的　以多普勒超声时间平均流速 (VTI/t)法和二维超声单平面Simpson法为对照 ,探讨经胸三维彩色多普勒超声心动图测量心搏出量 (SV)的可行性和准确性。方法　随机选择接受超声心动图检查的临床患者 3 5例 ,分别采用时间平均流速法、单平面Simpson法和三维彩色多普勒法测定SV。 结果　时间平均流速法和单平面Simpson法的心搏出量测值与三维彩色多普勒超声心动图的测值差异无显著性意义 (均 P >0 .0 5 )。多普勒时间平均流速法与三维彩色多普勒超声心动图测定的SV中度相关 (r =0 .72 ,P <0 .0 0 1) ;单平面Simpson法与三维彩色超声测值间高度正相关 (r =0 .82 ,P <0 .0 0 1)。直线回归分析表明 ,多普勒时间平均流速法和三维彩色多普勒超声心动图间的回归系数为 0 .5 49,决定系数R2 为 0 .5 2 ,回归方程为Y =0 .5 49X +2 7.2 4(P <0 .0 1)。如果以单平面Simpson法为自变量 ,以三维彩色超声法为因变量 ,其回归系数为 1.0 2 ,决定系数 R2 为 0 .67,回归方程为 Y =1.0 2 X -1.63 (P <0 .0 1)。结论　采用三维彩色多普勒超声心动图测量心搏出量具有高度的可行性和准确性。Objective To evaluate the clinical feasibility and accuracy of determination of stroke volume by transthoracic three-dimensional color Doppler echocardiography compared with the single-plane Simpson method and the two-dimensional pulse wave echocardiography(2D-PW) integral method in clinic.Methods The study population consisted of 35 randomized patients [including 27 male and 8 female,mean age (42±17) years].The Agilent Sonos 5500 echocardiography imaging system was used to perform conventional two-dimensional echocardiographic examination and acquire raw two-dimensional images.The three-dimensional reconstruction was performed using TomTec GmbH three-dimensional image reconstruction workstation which was equipped with EchoView 4.2 software.Results The measurement value of stroke volume by 2D-PW integral method,single-plane Simpson method were very close to that by three-dimensional color Doppler echocardiography( P > 0.05 ).Linear correlation analysis indicated that the stroke volume determined by 2D-PW integral method and single-plane Simpson method correlated well with that determined by three-dimensional color Doppler echocardiography ( r = 0.72 , 0.82 ,respectively, P < 0.01 ).Conclusions The three-dimensional color Doppler echocardiography can determine stroke volume of left ventricle feasibly and precisely in clinical patients.
