免疫性血小板减少性紫癜免疫学改变与中医证候分型的关系  

Analysis of the Correlations between Immunological Changes and Syndrome Groups of Patients with Immunic Thrombocytopenic Purpura (ITP)

摘　　要：为探讨免疫性血小板减少性紫癜(ITP)中医证候分型与免疫学改变关系,应用APAAP桥联酶标技术和双抗夹心ELISA法,测定了66例ITP患者与53名健康对照者外周血T淋巴细胞亚群(T亚群)、成熟B淋巴细胞、NK细胞、血小板相关抗体(PAlg)和血浆抗血小板膜糖蛋白自身抗体,分析了它们与中医辨证分型、年龄、血小板计数、病程和免疫球蛋白间的关系。结果显示:T亚群和PAlg的改变与中医证型密切相关。按血热妄行型→气血两虚型→脾肾阳虚型→肝肾阴虚型→阴阳两虚型的顺序,T亚群和PAlg改变逐渐明显,年龄与病程依次递增,和各型轻重程度相吻合。中医各证型间不仅差异显著,还各有特点。证实ITP中医辨证分型的客观性和科学性。To study the relationship between the immunological changes and syndrome (Zheng, 证) groupsby TCM of ITP, the T-lymphocyte subsets, B-lymphocyte, NK cell, platelet-associated IgG (PAIgG,PAIgA, PAIgM) and antiplatelet-autoantibodies (GPIIb, GPIIIa, GPIb) of 66 patients with ITP wereassied using APAAP and ELISA method separately. It was found that the T-lymphocyte subsets,PAIg and syndrome groups of ITP were closely related. From the group of blood-heat (Xuere-wangxing,血热妄行) to the group of deficiency of both Qi and blood (气血两虚), the group of astheniaof both Spleen and Kidney (脾肾两虚), the group of deficiency of Liver-yin and Kidney-yin (肝肾阴虚),and the group of dificiency Yin and Yang (阴阳两虚) Ts lymphocyte successfully increased (from29. 0±8.0% to 47.2±10.0%), Th/Ts ratio declined (from 1.35±0.60% to 0.69±10%), PAIgincreased gradually except for PAIgM,PAIgG of the group of deficieny Yin and Yang (阴阳两虚).Only the Th of the group of asthenia of both Spleen and Kidney among 5 syndrome groups wasdecreased significantly and contrary to the group of deficiency of Liver-Yin and Kidney-Yin. Theseresults indicated that every syndrome group has specific characteristics,and immunological changesof ITP could have prognostic value.
