清热止血药治疗乳糜尿前后足背皮肤电镜观察及其发病与治疗机理探讨

摘　　要：为了探讨清热止血药治疗乳糜尿机理,本组选择住院患者10例,经清热止血药治疗45夭,分别于治疗前后取材足背皮肤进行电镜比较观察。结果发现,治疗前破损严重的毛细淋巴管壁,治疗后修复完好;治疗前毛细血管内皮细胞的饮液小泡密集,治疗后明显减少;治疗前肥大细胞稀少而形态变异为梭形,治疗后明显增多,形态为圆形或椭圆形,核大,所含颗粒多而体积大;治疗前有淋巴水肿,治疗后消失。结果表明,淋巴管壁的广泛损伤为乳糜尿的发病机理,修复损伤管壁为其治疗机理。在修复过程中,肥大细胞可能具有工兵样作用。Heat-clearing and hemostatic drugs were effective in treating chyluria. In order to explore the Gurative mechanism, a group of 10 in-patients with chyluria were chosen and treated with heat-clearing and hemostatic drugs for 45 days. Some pre- and post- treatmentally skin from dorsum pedisof the patients were taken off and observed under electron microscope. It showed that the walls oflymphatic capillaries were seriously damaged berore treatment and rehabilitated wall after treat-ment. The pinocytotic vesicles were packed tightly in the endothelial cells of capillaries beforetreatment and their number was decreased remarkably after treatment; the mast cells were rare innumber and abnormal as spindle shape before treatment and they increased in number and becameround or oval in shape, the nucleus became large, the granules they contained became more innumber and larger in size after treatment; lymph edema was noticed before treatment and itdisappeared after treatment. The result showed that the extensve lesion of lymphangial walls is the pathogenesis of chyluria, while the rehabilitation of them is the curative mechanism. It seems thatin the process of rehabilitation the mast cells act took the role of 'soldier-engineer'.
