摘  要:8例平均年龄为25.8岁的青壮年病人因患膝关节疼痛及功能紊乱进行关节镜检查,发现有内侧或外侧半月板游离薄缘的磨损及微小破裂。病变部位作选择性半月板部分切除术。切除标本经制备以后在透射电镜下观察,发现:1.细胞成分的变化,包括软骨细胞及成纤维细胞的明显退变,以及正常情况下不存在的肌成纤维细胞的出现。2.基质成分的变化,包括胶原纤维排列的杂乱无章及纤丝的断裂。弹力纤维变得更成熟,并发生弹力纤维融合,成为弹力纤维块。这些半月板破裂虽然轻微,但病理变化已经非常明显。In eight patients with an average age of 25.8 years, arthroscopic examination was performed for painful dysfunction of knee joint. Attrition and minute tear were found over the thin and free edge of either the medial or the lateral meniscus. Selective meniscectomy of the lesioned areas was carried out and the excised specimens were studied under transmission electron microseope, revealing the following changes. (1) Changes on the part of the cellular components included overt degeneration of the chondrocytes and the fibrobiasts and emergence of myofibroblasts, which were not present in normal meniscus. (2) Changes on the part of the matrical components included haphazard arrangement and fragmentation of the collagen fibers. The elastic fibers became more mature. There was also abnormal fusion of the mature elastic fibers with interlink of the central materials. Sometimes many elastic fibers could even fuse together into a big lump. Although the lesions in these menisci were minute and early, the pathological changes were already very marked. It was thus rational to carry out arthroscopic examination and selective surgery for these early lesions over the thin and free edge of the menisci, which defied any possibility of spontaneous healing.
