急性脑外伤患者内源性促甲状腺素释放激素含量改变的临床意义

摘  要:作者采用放射免疫测定法(RIA)检测了29例急性脑外伤患者脑脊液(CSF)和血浆中免疫活性促甲状腺素释放激素(ir—TRH)的含量变化,并探讨其临床意义。发现受伤当天轻型脑外伤组(n=12)CSF和血浆中的ir—TRH含量(pmol/L)分别为:81.90±24.98和84.81±23.13,中型脑外伤组(n=9)分别为:161.78±32.82和182.76±23.99重型脑外伤组(n=8)分别为196.25±21.22和239.82±22.18,而对照组(n=10)分别为:70.31±33.18和78.12±31.89。经动态观察发现,随着病情改善,CSF和血浆中的ir—TRH含量也逐渐恢复到对照组水平,而预后差的患者,其ir—TRH含量显著下降,并明显低于对照组。提示本检测对指导脑外伤的治疗和判断预后具有一定意义。The contents of immunoreactive thyrotropin-releasing hormone (ir-TRH) in cerebrospinal fluid (CSF) and Plasma were determined after acute head trauma in 29 patients, using a sensitive method of radioimmunoassay (RIA). The results showed that, the concentrations of ir-TRH (pmol/L) in CSF and plasma at the injured day were 81.90±24. 98 and 84.81±23.13 in minor (n=12), 161.78±32.82 and 182.76±23.99 in moderate (n=9), and 196.25±21.22 and 239.82±22.18 in severe (n=8), respectively, While the contents of it-TRH in CSF and plasma were 70.31±33.18 and 78.12±31.89 in control groupp (n=10). Dynamic observations revealed that, the contents of ir-TRH in CSF and plasma were gradually recovered to the normal leveis with the improvemnt of head-injured patients, whereas they were significantly decreased and much lower than normal levels in patients with poor prognosis. These results suggest that the assay of ir-TRH in CSF and plasma have clinical significance in reflecting the severity and prognosis of acute head-injured patients.
