肥厚型心肌病左室流出道压力与左房大小的关系对比

摘　　要：目的　应用超声心动图分析肥厚型心肌病 (hypertrophic cardiomyopathy,HCM)左室流出道压力阶差 (L VOTPG)与左心房 (L A)大小的关系。方法　随机选择肥厚梗阻性心肌病 (HOCM) 4 5例、肥厚型非梗阻性心肌病 (HNOCM) 4 0例、正常人 5 2例 ,分别测量左室流出道 (L VOT)宽度、左室流出道压力阶差(L VOTPG)及左心房的大小。结果　正常对照组 L VOT的宽度 1 8.2 mm,L VOTPG 8.76 mm Hg,L A31 .5 mm;HNOCM组 L VOT的宽度 1 5 .1 8mm,L VOTPG 1 3.1 3mm Hg,L A 39.1 2 mm;HOCM组 L VOT的宽度 9.2 8mm,L VOTPG 38.74 mm Hg,L A 4 3.2 1 mm。结论　 HOCM患者 L VOTPG对左房功能的影响较 HNOCM患者大 。Objective To analyze the relationship between the pressure gradient of left ventricular out Tract(LVOTPG)and the width of left atrium(LA)in hypertrophic cardiomyopathy(HCM)using echocardiography.Methods Fourty five cases with hypertrophic obstructive cardiomyopathy(HOCM),40 cases with hypertrophic nonobstructive cardiomyopathy(HNOCM)were randomized selected and 52 subjects selected as normal control group.The width of LVOT,LVOTPG and the width of LA were meausred in all the 3 groups.Results The width of LVOT,LVOTPG and LA width in HOCM,HNOCM and normal groups were 9 28 mm,38 74 mm Hg,43 21 mm;15 18 mm,13 13 mm Hg,39 12 mm;18 12 mm,8 76 mm Hg,36 5 mm respectively.Conclusions The influence of LVOTPG on the left atrial function in HOCM is more significant than that of HNOCM group,and the most significant than that of the control group(All P <0 05).
