摘 要:目的 探讨小儿脊髓栓系综合征 (tetheredcordsyndrome ,TCS)的螺旋CT(SpiralCT ,SCT)影像学特点及诊断价值。方法 回顾性分析手术确诊的TCS患儿 13 6例 ,对其SCT平扫及多平面重建 (multiplannerreformation ,MPR)的影像特点与临床表现、手术所见进行对照分析。结果 13 6例TCS患儿中圆锥均低于L3 甚至达S2 以下 ,合并脊髓骨性纵裂 2 4例、脂肪瘤 61例、终丝增粗脂肪变 11例、脊髓空洞症 9例、椎体畸形 2 6例。 13 6例TCS患儿伴有脊柱裂 12 9例 (95 % ) ,其中包括 18例隐性脊柱裂 ,111例显性脊柱裂。通过SCT/MPR均能获得较满意的图像。结论 SCT/MPR能为手术提供完整的资料 ,是继MRI。Objective To study the features and diagnostic value of spiral CT multiplanner reformation(SCT/MPR) in children with tethered cord syndrome(TCS).Methods The imaging characteristics of SCT/MPR were analysed and compared with clinical features and operative findings in 136 children with TCS . Results Conus medullaris of 136 children with TCS were all terminated bellow L 3 or even S 2 level.There were 24 cases with diastematomyelia ,61 cases with intraspinal canal lipoma,11 cases with thickened filum terminale, 9 cases with syringomyelia and 26 cases with vertebral deformity in 136 cases.129 cases(95%) were in company with spina bifida,of them,18 cases with occult spina bifida and 111 cases with open spina bifida. The features of TCS were showed clearly in all cases by SCT/MPR. Conclusion SCT/MPR can supply the complete imaging data for surgery in children with TCS and it is an other important method in diagnosing TCS except use MRI and sonography.
