左氧氟沙星对复治涂阳合并肝功能异常结核病人疗效及安全性观察

摘　　要：目的 :观察左氧氟沙星联用非利福霉素类一线抗痨药对初治失败 ,痰菌阳性且服利福霉素类药物出现严重肝功能损害或利福平类过敏而不能使用的复治肺结核病人之疗效 ,全面评价左氧氟沙星的抗结核作用。方法 :入选 10 2例病人均为复治涂阳肺结核病人 ,随机分配成治疗组 5 3例 ,予 2H3 VE3 Z3 S3 / 6H3 VE3 方案抗痨 ;对照组 4 9人 ,予2H3 R3 E3 Z3 S3 / 6H3 R3 E3 方案抗痨。观察痰菌阴转、X线好转情况及药物副反应。结果 :痰菌阴转率治疗两个月时治疗组及对照组分别为 5 6 6 %和 5 9 1% ,疗程结束时痰菌阴转率分别为 83 0 %和 85 7%。疗程结束时X线病灶有效率依次为 86 8%、87 8% ,治疗组与对照组相比无显著差异性。治疗组未见严重不良反应。结论 :左氧氟沙星具有良好抗结核作用 ,是一种使用安全、副反应小的新一代抗结核药 ,适用于复治病人及有肝功能障碍的初治病人。Objective:To observe the clinical efficacy and safety of Levofloxcin combined with other antituberculosis drugs (without RFP) in treating patients of pulmonary tuberculosis who were smear positive and had allergic reaction to RFP or hepatic function injury, and to evaluate the activities of Levofloxcin against Mycobacterium tuberculosis. Methods:Both of the 102 cases are smear positive and re-treated pulmonary tuberculosis patients. The patients were randomly allocated into treatment group(2H 3VE 3Z 3S 3/6H 3VE 3) and control group(2H 3R 3E 3Z 3S 3/6H 3R 3E 3) for observing the sputum negative conversion rate or X-ray resolution rate and the side effects of drugs. Results: The sputum negative conversion rates at the second month in the treatment group and the control's were 56.6% and 59.1% respectively,and at the end of the chemotherapy were 83.0% and 85.7%. X-ray resolution rate at the end of the chemotherapy were 86.8% and 87.8%.There was no significant difference between two groups.There was no severe side effects in the treatment group. Conclusion: Levofloxcin showed good efficacy.It was safe and few side effect in antituberculosis chemotherapy,and it could be used for re-treated pulmonary tuberculosis patients or new case with hepatic function injury.
