摘  要:目的 探讨针刺对急性脑梗死的治疗作用及其机制。方法 将 3 2例急性脑梗死患者随机分为针刺组和对照组 ,每组各 16例。针刺组接受针刺 (每周 5次 ,共 3周 )和常规治疗 (静脉滴注丹参及对症支持治疗等 ) ,对照组仅接受常规治疗。两组患者均在治疗前、后采用改良爱丁堡 斯堪的维亚脑卒中评定量表(MESSS)评估神经功能 ,检测磁刺激运动诱发电位 (MEP)、短潜伏期体感诱发电位 (SLSEP) ,并测定血清神经元特异性烯醇化酶 (NSE)和髓鞘碱性蛋白 (MBP)含量。结果 治疗 3周后 ,针刺组患者MESSS评分、血清NSE及MBP含量明显降低 ,且MEP中枢运动传导时间 (CMCT)、SLSEP中枢感觉传导时间 (N13~ 2 0 IPL)明显缩短 ,与对照组比较差异均有显著性意义 (P <0 .0 5)。结论 针刺能促进急性脑梗死患者神经功能的康复 ,其机制可能与其保护神经元受损。Objective To explore the therapeutic ef fe cts and the mechanism of acupuncture on patients with acute cerebral infarction. Methods Thirty-two patients with acute cerebral i nfarction were recruited and randomly divided into an acupuncture group (group 1 ) and a control group (group 2). Acupuncture treatment (5 times a week, for 3 we eks) was conducted solely in patients of group 1, in addition to the routine tre atment (including intravenous drop of Dan Sen injection and supportive treatment ) for both groups. The neural functions of all patients were assessed by use of improved MESSS assessment, while magnetic stimulation motor evoked potential (M EP),short latency somatosensory evoked potential (SLSEP), and the concentrations of serum neuron-specific enolase (NSE) and myelinbasic protein (MBP) were meas ured before and after treatment. Results Compared w ith group 2, MESSS, concentrations of serum NSE and MBP in group 1 decreased dr amatically, and central motor conduction time (CMCT), N 13~20 interpeak la t ency (IPL) shortened markedly 3 weeks after treatment. The difference between t he two groups was significant ( P <0.05). Conclusion Acupuncture could promote the recovery of neural function of patients wit h acute cerebral infarction, and the mechanism might be related to the protect ion of the neuron and regulation of the excitability of central sensory and mot or pathway.
