摘 要:目的 :探索骨代谢指标在诊断骨质疏松症 (Osteoporosis ,OP)中的临床价值。 方法 :对骨科门诊自愿受试者进行跟骨定量超声 (Quantitativeultrasound ,QUS)测值并用酶联免疫法 (Enzymelinkedimmunosorbentassay ,ELISA)测定血中骨钙素 (BoneGlaprotein ,BGP)与吡啶酚 (Pyridinium ,PYD)。随机取绝经后女性 5 1例作为研究对象 ,据QUS值分成骨量正常组 (18例 )、骨量减少组 (18例 )与骨质疏松组 (15例 ) ,采用SPSS软件对QUS相关值及作为骨代谢指标的BGP、PYD值进行数据处理 ,分析QUS测定值与骨代谢指标在骨质疏松中的相关性。结果 :3组年龄分别为 5 9.4 4± 6 .5 1、6 0 .88± 7.2 9、6 2 .0 9± 5 .87岁 ,各组间无明显差异(P >0 .0 5 )。OP组和骨量减少组的BGP、PYD均值均高于骨量正常组 (P <0 .0 5 ) ,其中OP组的PYD均值显著高于骨量正常组 (P <0 .0 1)。QUS中的宽波段超声衰减 (Broadbandultasoundattenuation ,BUN )值、骨硬度指数 (Stiffness ,STI)与BGP、PYD值均呈负相关 (P <0 .0 5 ) ,超声声速 (Soundofspeed ,SOS)与PYD呈负相关 (P <0 .0 5 ) ,而与BGP无显著相关性 (P >0 .0 5 )。结论 :BGP、PYD作为骨代谢生化指标 ,反映骨代谢的重建状态 ,在绝经后女性OP症中 ,其值升高提示为高转换型OP。Objective:To investigate the clinical significance of bone metabolic biochemical markers in diagnosis of ostenoporosis. Methods:The volunteers of the orthopedic outpatient service were checked by calcaneus Quantitative Ultrasound (QUS)and Bone glaprotein(BGP) Pyridinium(PYD) of bone metabolic biochemical markers.51 postmenopausal women were enrolled in this study randomly. They were divided into three groups:18 with normal bone density (group A),18 with osteopenia (Group B) and 15 with osteoporosis (Group C), according to QUS parameters ,The data were dealt with the software of SPSS to analyze the correlation between the main parameters of QUS and BGP,PYD. Results:The mean of ages in three groups were respectively 59.44±6.51,60.88±7.29,62.09±5.87 years old ,which were no significantly difference (P>0.05).BGP and PYD were significantly increased in Group C and B, compared with Group A (P<0.05). PYD in group C was remarkable higher than that in group A(P<0.01).BUA(Broadband ultrasound attnuation ) was negatively correlated with BGP(r=-0.61 ,P<0.05) and PYD(r=-0.65,P<0.05),so was STI(stiffness index) (r=-0.55,P<0.05,r=-0.63,P>0.05),SOS (sound of speed ) was negatively correlated with PYD(r=-0.67,P<0.05), and not correlated with BGP (r=-0.22,P>0.05) Couclusions:Being as bone metabolic biochemical markers,BGP adn PYD would reflect reconstructive condition in bone metabolism.The elevation of them was thought of that OP in postmonopausal women would belong the OP in the type of the high turnover. Therefore BGP and PYD would have positive clinical value for diagnosing OP or using as complement of OP diagnosis to learn about OP and to direct prevention from and treatment of OP.
