摘  要:目的 探讨谷氨酰胺对烧伤大鼠肠三叶因子 (intestinaltrefoilfactor,ITF)表达的影响及其可能的机制。方法 采用 3 0 %体表面积Ⅲ度烧伤大鼠模型 ,随机分正常对照 (C)组 ,烧伤后普通肠道营养 (EN)组及烧伤后添加谷氨酰胺的肠道营养 (GLN)组。EN组用等量甘氨酸补足氮量 ,使两组烧伤大鼠保持等氮、等热卡 ,动态观察伤后两组大鼠ITF及ITFmRNA的变化。结果 正常大鼠小肠中ITF及ITFmRNA均有一定表达 ,烧伤后其表达一过性增加 ,随后明显降低。两组比较 ,GLN组大鼠肠组织中ITF及ITFmRNA水平明显高于EN组 ,同时GLN组肠粘膜受损程度也明显低于EN组。结论 严重烧伤后肠粘膜结构受损是ITF合成下降的主要原因 ,谷氨酰胺能保护肠粘膜 ,维护杯状细胞的正常结构和功能 。Objective To explore the effects of glutamine on intestinal trefoil factor (ITF) and ITF mRNA expression in burned rats and the possible mechanisms. Methods Wistar rats inflicted with 30% total body surface area full thickness burns were randomly divided into three groups: normal enteral nutrition (EN), enteral nutrition plus glutamine (GLN), and normal control (C). The expression and content of ITF in the intestine were quantitatively and localizationally determined by in situ hybridization (ISH), immunohistochemistry, and high performance liquid chromatography (HPLC). Results ITF and ITF mRNA were expressed in the whole small intestine, and most of them were localized in goblet cells of intestinal villus. After burn injury, the structure of intestinal mucosa was severely damaged, and the abilities of goblet cell synthesis and ITF secretion, especially ITF dimer, were significantly decreased. GLN could lessen the extent of ITF drop though it could not reverse this trend. Meanwhile, compared with EN, GLN could abate the intestinal mucosa injury obviously. Conclusion The decline of ITF synthesis may result from damaged intestine structure after severe burn. GLN can protect the intestinal mucosa, preserve the normal structure and function of goblet cells, and promote the synthesis and secretion of ITF, particularly ITF dimer.
