心脏组织结构的二次谐波显像——灰阶定量研究

摘  要:目的 :通过计算机辅助的声学组织灰阶定量技术评价二次谐波显像在不同切面、不同深度心肌与基波显像的差别。方法 :采用美国HewlettParkardImagePointHX心脏超声诊断仪和 2 - 4MHz探头。在增益、抑制、TGC和LGC不变的情况下 ,对 2 0例患者 ,分别用基波显像和二次谐波显像采集胸骨旁左室长轴切面、左室二尖瓣水平短轴切面及心尖四腔心切面。用Windows 98支持下的Photoshop 6 .0软件对图象进行分析。结果 :示二次谐波显像后近场的前间隔灰阶均值、离散度、中位数、最小值、最大值及优势灰阶分别高于基波显像 19%、31%、2 4 %、10 %、15 %和 32 % ;远场的左室后壁的心肌灰阶均值、离散度、中位数、最小值、最大值及优势灰阶分别高于基波显像 2 1%、4 5 %、2 1%、17%、2 5 %和 4 6 %。进一步研究发现二次谐波显像后左室侧壁的心肌灰阶均值、离散度、中位数、最小值、最大值及优势灰阶心肌灰阶各测值低于基波显像 12 %、4 7%、11%、16 %、19%和 30 %。结论 :二次谐波显像后心脏结构的轮廓更为清晰 ,尤其在透声条件不佳的情况下 。Objective: To evaluate the difference between second harmonic imaging and foundamental imaging in cardiac structure at different view and different depth. Methods:20 patients were studied by computer-assistant quantitative analysis of myocardial echodensity. The parasternal left ventricular long axis view, left ventricular short axis view at mitral valve level and apical four chamber view were obtained with Hewlett Pardkard Image Point HX Echocardiographic System and 2~4MHz transducer. The gray-scales were analyzed by Adobe Photoshop6.0 software supported by Microsoft Windows 98. Results: The second harmonic imaging was successfully obteined in all patients. After second harmonic imaging, the mean gray-scale, standard deviation, median, minimal value, maximal value and superiority gray-scale of anterior septum in near area were 19%,31%,24%,10%,15% and 32% higher than those of foundamental imaging. The mean gray-scale, standard deviation, median, minimal value, maximal value and superiority gray-scale of left ventricular posterior wall in far area were 21%,45%,21%,17%,25% and 46% higher than those infoundamental imaging. However, The mean gray-scale, standard deviation, median, minimal value, maximal value and superiority gray-scale of left ventricular latrial wall were 12%, 47%, 11%, 16%, 19% and 30% lower than those of foundamental imaging.Conclusion: Cardiac structure is very clear after second harmonic imaging ,especially imaging quality is bad, the echodensity of endocardium was enhanced.
