摘　　要：目的 评价经阴道彩色多普勒超声在诊断剖宫产子宫疤痕妊娠中的价值。方法 研究组22例,对照组50例,观察孕囊位置、妊娠物与子宫切口处肌层的回声、血流分布及两者的分界、宫腔宫颈情况。测量切口疤痕处肌层厚度及多普勒检测流速曲线分析。结果 剖宫产后62.4％的子宫下段切口肌层处由粘膜层向浆膜层呈楔形凹陷缺损。剖宫产子宫疤痕妊娠的声像图特征为妊娠物位于子宫前壁峡部剖宫产疤痕处,妊娠物与膀胱间的子宫肌层变薄,与子宫切口处肌层之间分界不清,回声紊乱,血流丰富,呈低速低阻型流速曲线,宫颈形态正常。结论 经阴道彩色多普勒超声在诊断子宫疤痕妊娠和判断治疗效果中起了重要作用。Purpose To evaluate the value of color Doppler transvaginal ultrasound in the diagnosis of eetopic pregnancy in previous caesarean section scar. Methods Twenty-two cases of eetopic pregnancy and 50 eases of eontrals were included in this study. The place of gestational sac, relation between caesarean section scear and the gestational sac, distribution of color Doppler flow signals, uterine cavity and cervix were observed. Results A wedged defect in the lower uterine myometrium following previous lower segment caesarean section were observed on 62.4% of patients. The ultrasonography characteristics of ectopic pregnancy in the caesarean section sear were that the gestational sac laeated in the lower part of the anterior uterine wall, and a thin myometrium between the bladder and the gestational sac. There was no margin between the gestatiorai sac and myometrium with heterogeneous, rich flow signal and low velocity, low resistance index wave form, and a normal cervix. Conclusions Color Doppler transvaginal ultrasound is of high value in the diagnosis of ectopic pregraney developed in previous caesarean section scar and in the evaluation of treatment response.
