摘  要:目的:探讨腰椎体终板及邻接骨髓变性的MRI表现。材料和方法:回顾性分析MRI诊断终板邻接骨髓变性者57例,100个终板,常规SE序列矢状位T1WI和T2WI,根据信号特点分为4个类型。结果:57例中,Ⅰ型(T1WI上呈低信号,T2WI上呈高信号)21例,34个终板;Ⅱ型(T1WI上呈高信号,T2WI上呈等或稍高信号)24例,37个终板;Ⅲ型(T1WI及T2WI上均呈低信号)113例,16个终板;Ⅳ型(混合型)10例,13个终板。Ⅰ、Ⅱ型好发于40-60年龄组,Ⅲ型与椎体Schmorl结节密切相关,Ⅳ型与椎体压缩及外伤有关。68%的退变终板好发于L4,5,61%呈条带形,62%贯穿终板全长,98%合并邻近问盘退变。结论:腰椎体终板及邻接骨髓变性是椎间盘退变的一个特殊类型。MR成像有助于分析终板及邻接骨髓变性的信号类型、分布特点及相关因素。Purpose: To investigate the MRI features of degeneration in bone marrow adjacent end plates (DMEP) of lumbar vertebral body. Materials and Methods: Fifty - seven patients with 100 DMEP were observed using MR sagittal T1WI and T2WI. According to MR features,DMEP divided into Ⅰ-Ⅳ types. Results: In 57 patients with 100 DMEP, 21 patients with 34 DMEP (hypointensity on T1WI and hyperintensity on T2WI) as type I ,24 patients with 37 DMEP (hyperintensity on T1WI and isointensity or slightly hyper-intensity on T2WI) as Ⅱ type, 13 patients with 16 DMEP (hypointensity on T1 WI and T2 WI) as Ⅲtype, 10 patients with 13 DMEP (heterogeneous signal intensity on T1 WI and T2 WI) as Ⅳ type. Type Ⅰ,Ⅱ most likely occurred the 40 - 60 age - specific, type Ⅲwere closely related to the vertebral Schmorl' node, type Ⅳwere related to collapsed vertebrae and trauma. 68% of EPD were found at L4.5level, 61% of DMEP were tenioid shape, 72% of DMEP height were range from 2mm to 5mm, 62% of DMEP was seen involving the whole endplate ,98% of with the neighboring inter vertebral disc degeneration. Conclusion: MRI findings of DMEP of lumbar vertebral body were helpful for analyzing its signal type, distribution feature and correlation factor.
