补肾生髓法治疗绝经后骨质疏松症的临床研究

摘　　要：目的 比较补肾生髓法和性激素替代疗法治疗绝经后骨质疏松症的疗效。方法 95例绝经后骨质疏松症患者随机分为三组:中药组(32例)口服补肾生髓汤,西药组(34例)口服倍美力(1粒/d)和安宫黄体酮(2.5mg/d),对照组(29例)不作特殊治疗;相隔半年,以肾虚证为主证观察评分,检测血清性激素水平、血钙、血磷、肝肾功能及骨密度(BMD),然后进行自身前后比较。结果 中、西药组治疗后可有效地缓解肾虚证的各大主症,使肾虚证评分明显下降(P<0.01),血清雌、孕激素水平较治疗前均有提高(P<0.01),且组间比较,两种治疗对雌、孕激素的影响无差异,中、西药组治疗前后腰椎及左股骨近端骨密度均无变化,但对照组两次检测骨密度水平呈下降趋势(P<0.05)。结论 补肾生髓汤和性激素替代治疗对绝经后骨质疏松症的肾虚症状有较好的缓解作用;补肾生髓汤具有植物雌、孕激素样作用,可以提高血清雌、孕激素水平,故有雌、孕激素替代作用,还可避免周期性阴道出血、乳腺疼痛等副反应;补肾生髓汤和性激素替代治疗均不能有效提高骨密度,但可以阻断骨密度下降的进程。Objective To compare the curative effect of the Bu Shen Sheng Sui principle with the hormone replacement therapy, which were used to cure the postmenopausal osteoporosis. Methods 95 postmenopausal osteoporosis sufferers were divided randomly into three groups. The Chinese herbal medicine group (32 cases) takes Bu Shen Sheng Sui soup orally. The western medicine group (34 cases) takes the oral conjugated estrogen (1#/d) and medroxyprogesteronum (2.5 mg/d). And the control group (29 cases) do not make any special treatment. Half a year later, observe and give marks, regarding kidney deficiency syndrome as the main symptoms, and examine the serum hormone's level, blood calcium, blood phosphorus, liver kidney function and bone mass density (BMD). Then make a contrast with that before treatments. Results After the Chinese medicine and the western medicine's treatments, each main symptoms of the kidney deficiency syndrome can be alleviated availably. There was significant difference (P<0.01). Besides, the serum estrogen and the progesterone will improve after curing. And a comparison between the two kinds of treatments shows that the two kinds remedies makes no different influence on the patients' estrogen and progesterone. The BMD of the entrance point of screw and the femoral have no variety. But regarding the two examinations in the control groups, the level of the BMD may report to a decline trend (P<0.05). Conclusion Bu Shen Sheng Sui soup has the plant estrogen and progesterone kind function, it can raise the level of serum estrogen and progesterone hormone, so it may take the place of the estrogen and the progesterone. It can also avoid side effects such as the periodic vagina bleeds, mammary glandsache, etc. and averting the otiose panic in patients. Bu Shen Sheng Sui soup and hormone replacement can't improve the BMD effectively, but it can break the progress of the BMD's descent.
