摘  要:目的 : 探讨HPV16 18阳性的尖锐湿疣DNA倍体和cyclinD1、P5 3、P2 1WAF1 蛋白表达的改变。方法 : 使用组织芯片技术和DNA计算机图像分析方法观察 2 2 7例外阴尖锐湿疣和 2 1例外阴鳞状细胞癌的DNA倍体类型 ,并进行cyclinD1、P5 3、P2 1WAF1 免疫组化研究。结果 : HPV 16 18阳性外阴尖锐湿疣 2C细胞占 (4 4 .36± 6 .6 6 ) % ,3C~ 4C占 (37.72± 8.4 3) % ,5C占 (15 .2 1± 6 .33) % ,>5C占 (2 .71± 1.0 4 ) %。 98例HPV16 18阳性尖锐湿疣中 ,cyclinD1、P5 3、P2 1WAF1 阳性者分别为 39例 (39.80 % )、5 2例 (5 3.0 6 % )、4 7例(4 7.96 % )。HPV 16 18阳性外阴尖锐湿疣与阴性者差异有显著性 (P <0 .0 1)。结论 : HPV16 18阳性外阴尖锐湿疣DNA倍体图像分析显示高度增殖活性 ,cyclinD1、P5 3高表达 ,P2 1WAF1Objective: To determine the changes in expression of DNA ploidy and cyclin D1, P53, P21 WAF1 protein in HPV16/18 positive vulva condyloma acuminatum (CA). Methods: The expression of DNA ploidy and cyclin D1, P53, P21 WAF1 protein in 227 patients with vulva, CA and 21 patients with vulva, SCC were analyzed with DNA ploidy image analysis and immunohistochemical methods by tissue microassay. Results: The diploidy cell in HPV16/18 positive vulval CA was (44.36±6.66)%, 3C-4C (37.72±8.43)%, 5C (15.21±6.33)%, >5C was (2.71± 1.04)%. Out of 98 cases of HPV16/18 positive vulval CA, the expressions of cyclin D1, P53 and P21 WAF1 protein were in 39 cases (39.80%), 52 cases (53.06%), 47 cases (47.96%) respectively. There was a significant difference between the HPV16/18 positive vulva, CA and the HPV16/18 negative (P<0.01). Conclusion:It is shown by DNA ploidy image analysis that the proliferation activity on HPV 16/18 positive vulva, CA is higher. The positive expression of cyclin D1 and P53 protein in HPV 16/18 positive vulval CA was higher than that in HPV 16/18 negative vulval CA.
