助孕术治疗结果不同的患者子宫动脉及分支血流特征的比较

摘　　要：目的　应用经阴道彩色多普勒超声探讨胚胎移植前子宫动脉及分支血流与内膜接受性间的关系。方法　按照治疗结果 (受孕和未孕 )进行分组 ,对 94例接受辅助生殖技术治疗的不孕妇女子宫动脉及其分支的血流进行回顾性比较分析。结果　胚胎移植前 ,子宫动脉、弓状动脉、放射动脉的各项多普勒血流参数在两组间差异无显著性意义。而螺旋动脉阻力指数 (RI)、收缩期 /舒张期血流速度比值 (S/D)的差异具有非常显著性意义 (0 .62± 0 .0 4对 0 .68± 0 .0 4,P <0 .0 0 1;2 .66± 0 .3 3对 3 .19± 0 .3 9,P <0 .0 1) ;搏动指数 (PI)的差异具有显著性意义 (1.15± 0 .17对 1.3 4± 0 .2 2 ,P <0 .0 5)。当RI >0 .72 ,PI >1.6,S/D >3 .6时 ,无一例妊娠。子宫肌层、内膜下彩色多普勒血流信号的有无和多寡在两组间差异也有显著性意义。结论　内膜局部的血流灌注较子宫动脉的血流更能准确反映内膜的接受性 。ObjectiveTo study the differences in blood flow characteristics of uterine artery and its branches between conception and non-conception women. MethodsThe blood flow of uterine, myometrial and subendometrial vessels was evaluated retrospectively in 94 infertile women who had undergone assisted reproductive techique(ART) treatments with different outcomes. ResultsThere were no significant differences in hemodynamic parameters of uterine, accurate, radial artery between conception and non-conception groups before embryo transfer(ET). The resistance index(RI), systolic/diastolic ratio(S/D), pulsatility index(PI) of spiral artery were significantly lower in those who achieved pregnancy as compared with those who did not: 0.62 ± 0.04 vs 0.68 ± 0.04 (P< 0.001 ), 2.66 ± 0.33 vs 3.19 ± 0.39 (P< 0.01 ), and 1.15 ± 0.17 vs 1.34 ± 0.22 (P< 0.05 ), respectively. Furthermore, when RI> 0.72 ,PI> 1.6 ,S/D> 3.6 , no pregnancy occurred. There also were differences in subendometrial and intra-endomentrial color mapping between conception and non-conception women. ConclusionsThese data suggest that the subendometrial and intra-endometrial blood flow perfusion, rather than the global blood flow of uterus is more important in predicting endometrium receptivity before ET.
