多普勒组织成像在评价原发性高血压患者左室整体舒张功能时最敏感部位的选择

摘　　要：目的:应用多普勒组织成像(DTI)测量二尖瓣环5个位点的运动参数,探讨二尖瓣环在评价左室整体舒张功能时最敏感位点的选择。方法:测量20例健康自愿者及60例原发性高血压患者二尖瓣环间隔、侧壁、下壁、前壁、后壁5个位点的Ea、Aa的DTI参数,计算Ea蛐Aa。结果:①高血压组二尖瓣环5个位点Ea、Ea蛐Aa明显减低(P<0.001);②正常组和高血压组二尖瓣环侧壁位Ea蛐Aa比值与平均比值相关系数最大(r=0.894,r=0.854)。结论:应用DTI测量二尖瓣环运动参数来评价左室整体舒张功能时,我们认为心尖四腔切面二尖瓣环侧壁位为最敏感位点。Objective: To investigate the optimal site of mitral annular motion by Doppler tissue imaging(DTI) in evaluating left ventricular diastolic function. Methods: Twenty healthy subjects and sixty patients with essential hypertension were studied by DTI. Five mitral annular sites, septal, lateral, posterior, inferior and anterior were adopted on apical 4-chamber, apical 2-chamber and apical long axis views. The Ea and Aa were measured, the Ea/Aa ratio was calculated. Results: ①All the essential hypertension patients had a decreased Ea and Ea/Aa ratio at all sites and Ea/Aa ratio was less than 1(P<0.001). ②In healthy subjects and essential hypertension patients, the correlation of the mean value of Ea and Ea/Aa ratio between the lateral site and five sites is the best(r=0.975, 0.960; r=0.912, 0.876). Conclusion: To evaluate left ventricular diastolic function by using DTI, we considered the lateral site of mitral annular on apical 4-chamber views was the best.
