摘 要:目的 通过序贯试验法测定罗比卡因颈丛神经阻滞的半数有效浓度 (EC50 )。方法 选择ASAⅠ~Ⅱ级 ,择期在罗比卡因颈丛神经阻滞麻醉下行颈甲状腺部分切除术患者 1 4例。罗比卡因浓度开始设定为 0 30 % ,其后浓度根据前一病人麻醉效果而序贯地按等比级数 1 2递增或递减。结果 计算得出EC50为 0 2 0 3%和 95 %可信区间 (95 %CI)为 0 1 78%~ 0 2 4 0 %。结论 序贯试验法简便、高效 ,适用于罗比卡因颈丛神经阻滞半数有效浓度测定。术前、术中辅助用药可能对其EC50 的影响有待进一步研究。Objective To determine the median effective concentration (EC 50 ) of ropivacaine for cervical plexus block for subtotal thyroidectomy. Methods\ Fourteen patients (ASA grade Ⅰ~Ⅱ) scheduled for selective subtotal thyroidectomy were randomly assigned into observation group to receive cervical plexus block by ropivacaine (the total volume was 30 ml for both sides). The concentration was determined by the response of a previous patient to a higher or lower concentration using up-and-down sequential allocation. The anesthetic efficacy was assessed as effective or ineffective. Results EC 50 was determined by the formula of W.J.Dixon & K.Brownlee, which was 0 203% and the 95% confidence interval (95% CI) was 0 178%~0 240%. Conclusions Up-and-down sequential allocation is a simple, efficient and reliable method for EC 50 determination of ropivacaine. The possible effects of assisted drug, before and during operation need further investigation.
