摘  要:目的 :观察柴苓汤在不同给药时间内对慢性不可逆性肾损害实验大鼠的治疗作用。方法 :2 1只雌性Wistar大鼠 ,一侧肾切除后尾静脉注射 5 0 0 μg单克隆抗体 1- 2 2 - 3。随机分为 3组 ,组 1给生理盐水作对照。组 2和组 3分别于造模后第 1d和第 7d开始每日腹腔注射柴苓汤 4 0 0mg/kg体重 ,共观察 6周。收集造模后第 3、7、14、2 1、35和 4 2d 2 4h尿 ,测量尿蛋白排泄 ;于第 4 2d处死全部大鼠 ,观察肾形态学、肾重量及血生化指标的改变。结果 :第2组大鼠尿蛋白排泄从造模后第 3d开始 ,第 3组大鼠尿蛋白排泄从造模后第 14d开始减少。在造模后第 4 2d ,给柴苓汤的两组大鼠肾重量低于对照组。光学显微镜结果提示 :柴苓汤减轻了肾小球形态学的损伤及肾小球系膜基质扩张。免疫荧光所见 :柴苓汤抑制了转化生长因子 β(TGF - β) ,α-平滑肌肌动蛋白 (α -SMA)和胶原Ⅰ在肾小球内的表达 ,并减少了ED1和CD8阳性细胞在肾小管间质和肾小球内的浸润。结论 :柴苓汤对大鼠慢性肾小球硬化具有防治作用。给药时间对柴苓汤的治疗作用有明显影响 ,提示早期治疗对防病。Objective:To observe the therapeutic effect of Chai-ling-tang on rat irreversible renal lesions in different time.Methods:Twenty-one female Wistar rats were randomly divided into three groups after injection of 500 μg monoclonal antibody (MoAb) 1-22-3 following uninephrectomy. Group 1 was treated with phosphate-buffered saline (PBS) as a control and group 2 and 3 were treated with Chai-ling-tang 400 mg/kg bodyweight on a daily basis beginning on day 1(group 2) and day 7(group 3) respectively. Urine was collected from the rats on days 3, 7, 14, 21, 35 and 42 after the injection of MoAb 1-22-3 into the uninephrectomized rats. The amount of urinary protein was determined. On day 42, all rats were sacrificed for blood biochemistry, immunofluorescent and light microscopy of the kidneys.Results:The urinary protein in Group 2 was suppressed on day 3, 7, 14, 21 and in Group 3 on day 21. The weight of kidneys in groups given Chai-ling-tang was also lower than that in the PBS control. Meanwhile, Light microscopy findings showed that Chai-ling-tang alleviated Glomerular morphological lesions and Mesangial matrix expansion. Immunofluorescent findings showed that Chai-ling-tang suppressed the expression of transforming growth factor-beta (TGF-β), alpha-smooth muscle actin (α-SMA) and collagen type Ⅰ in glomeruli and decreased the number of ED 1 and CD 8 positive cells in tubular-interstitium as well as in glomeruli.Conclusion:Chai-ling-tang blocked the progression of renal lesions in this model even when it was administered after the initiation of the disease. In addition, earlier treatment more effectively prevented the progression of the renal lesions.
