摘 要:目的 :评价电子束CT(EBT)及其三维重建技术对冠状动脉搭桥术后桥血管开通的诊断价值。材料和方法 :16例冠状动脉搭桥术后的患者行EBT血管造影和三维重建、血流扫描及曲线分析 ,其结果与手术结果对照。结果 :所有 5 3支桥血管均重建成功 ,其中乳内动脉桥 12支 ,大隐静脉桥 40支 ,桡动脉桥 1支。通畅的桥血管 3 8支 (67.9%) ,不同程度狭窄 6支 (11.3 %) ,完全闭塞 9支 (17.0 %)。 12支乳内动脉桥支 11支通畅 (91.7%) ,40支大隐静脉桥支 2 7支通畅(67.5 %)。1支桡动脉桥闭塞。EBT桥血管血流曲线及定量参数可辅助判断开通情况。结论 :EBT三维血管重建成像及血流扫描是评价冠状动脉搭桥术后桥血管开通的极有价值的无创性检查方法。Purpose: To assess usefulness of electron beam tomography (EBT) in non-invasive assessment of the patency of coronary artery bypass grafts. Materials and Methods: Sixteen consecutive patients who underwent coronary artery bypass graft surgery were scanned using both EBT angiography with 3-dimensional reconstruction and EBT flow study with time-density-curve analysis. The results of EBT were correlated with operatic findings. All patients didn’t undergo coronary angiography(CAG) after EBT scanning. Results: Of 53 bypass grafts successfully reconstructed by EBT workstation,including: internal mammary artery (IMA) grafts(12),saphenous-vein (SVG) grafts(40) and racial artery(RA)grafts(1). Thirty eight grafts(67.9%) were patent,six grafts(11.3%) were restenosis and 9 grafts(17.0%) were totally occlusive. One racial artery graft was occluded. EBT flow study could qualitatively evaluate patency. Conclusion: EBT three-dimensional reconstruction and flow study are noninvasive and effective method for examing patency of venous and arterial bypasses.
