Value of Auditory Brainstem Response and Distortion Product Otoacoustic Emissions in Hearing Screening of Neonates with Hyperbilirubinemia

摘  要:目的 探讨听性脑干反应 (ABR)及畸变产物耳声发射 (DPOAE)对高胆红素血症新生儿听力筛查的临床意义 ,为早期干预治疗提供依据。方法 对 170例高胆红素血症新生儿进行ABR和DPOAE检查 ,对其中部分异常者 6个月后复查。结果  170例患儿中 ,ABR和DPOAE的总异常率分别为 5 4 .12 %和 32 .6 5 % ;血清胆红素水平越高 ,ABR和DPOAE异常率越高 (P <0 .0 1) ,ABR异常越严重 (P <0 .0 1) ;胆红素浓度恢复正常后 6个月复查ABR和DPOAE ,恢复率分别为 77.12 %和 77.0 1% ,其中ABR异常DPOAE正常耳较两者均异常耳容易恢复 (P <0 .0 1)。Objective To evaluate the clinical efficacy of auditory brainstem response(ABR) and distortion product otoacoustic emissions(DPOAE) in hearing screening of neonates with hyperbilirubinemia for early treatment.Methods 170 neonates with hyperbilirubinemia who underwent ABR and DPOAE were enrolled and followed up for six months.Results In 170 neonates with hyperbilirubinemia, abnormal rates of ABR and DPOAE were 54.12% and 32.65% respectively, the level of serum bilirubin concentations was associated with abnormal rates of ABR, DPOAE ( P <0.01) and the ABR repectively( P <0.01); At six months follow up, the results showed that the rate of ABR and DPOAE resumption were 77.12% and 77.01%. The hearing with abnormal ABR and normal DPOAE was easier to be resumed compard to the one with abnormal ABR and DPOAE.Conclusion Our study demonstrated that combination of ABR and DPOAE in hearing screening of neonates with hyperbilirubinemia is more accurate and sensitive.
