摘 要:【目的】探讨CD134(OX40)及核因子-kB(NF-kB)在狼疮性肾炎(LN)发病机制中的可能作用。【方法】40例LN患者肾组织,用免疫组织化学方法检测CD134表达,用碱性磷酸酶-小鼠抗碱性磷酸酶免疫复合物法(APAAP)测定NF-kB表达,分析CD134与NF-kB的相关性。【结果】Ⅳ型和非Ⅳ型LN患者肾组织中,CD134表达都显著上调,灰度定量测定分别为肾小球4.22±1.65,2.43±1.32;肾小管7.72±2.69,4.46±2.41;肾间质为6.56±2.17,4.05±1.83,与正常对照组相比,P<0.01,P<0.05。Ⅳ型LN肾小球系膜细胞每横切面NF-kB阳性细胞数为13.1±8.2,肾小管上皮细胞NF-kB阳性小管数为38.5%±17.2%;非Ⅳ型LN肾组织则分别为3.2±1.8 NF-kB阳性细胞数/横切面,22.1%±9.8% NF-kB阳性小管数,与正常对照组相比,P均<0.05。Ⅳ型LN与非Ⅳ型LN比较,上述指标有显著差异,P均<0.05。除系膜细胞、内皮细胞和远曲小管CD134表达明显增强外,肾间质毛细血管和大血管内皮细胞亦有CD134表达阳性的浸润细胞(P<0.01)。LN患者肾小球和肾小管中CD134与NF-kB的表达呈正相关(分别为r=0.5542,P<0.05;r=0.6279,P<0.05)。【结论】LN患者存在CD134共刺激分子的异常表达。NF-kB在LN肾组织局部的表达明显增高。[Objective] To investigate the role of CD 134 (OX40) and NF-KB in the pathogenesis of lupus nephritis (LN). [Methods] Renal in situ CD 134 and NF-KB expression were examined in 40 LN patientbiopsy specimens by immunohistochemistry and microwave-based immunohistochemistry respectively. The relationship between expression of CD134 and NF-KB was analyzed. [Results] In class IV and non-class IV LN patients, the expression of glomerular , tubular and renal interstitial CD134( gray level ) were 4. 22 ±1.65, 2. 43 ±1.32, 7. 72 ±2. 69,4.46± 2. 41 and 6. 56 ± 2. 17,4. 05 ± 1. 83. It was higher than that of normal control, P < 0. 01, P < 0. 05 respectively. The expression of NF-KB in class IV and non-class IV LN patients were 13. 1±8. 2 NF-KB positive glomerular cell/ gcs, 38.5%±17.2% NF-KB positive tubular and 3. 2 ±1.8 NF-KB positive glomerular cell/ gcs, 22. 1% ± 9. 8% NF-KB positive tubular. It was also higher than that of normal control, all P < 0. 05 . The expressiom of it in class IV LN was higher than that of non-class IV LN group ( P < 0. 05), where there was intense staining of endothelial cell, distal tubules, and interstitial mononuclear cell. The CD134 expression of glomerular and tubular was closely related to NF-KB expression respectively ( r = 0. 554 2, P < 0. 05; r = 0. 627 9, P < 0. 05). [Conclusion] The abnormal expression of costimulato-ry molecule CD134 is well evidenced in LN. Strong expression of renal in situ NF-KB is likely mediat- ed by CD 134 signal pathway mediate activation.
