摘  要:探讨卡介菌多糖核酸对系统性红斑狼疮(SLE)患者外周血单个核细胞(PBMC)中Th细胞因子mRNA产生及其表达的影响,初步了解其作用机制。方法:将SLE患者随机分为处理组和对照组,于治疗前后采用逆转录-聚合酶链反应(RT-PCR)和ELISA技术分别检测Th1型细胞因子γ干扰素(IFN-γ)和Th2型细胞因子IL-4、IL-10的mRNA表达及PBMC培养上清水平,并评估疾病活动性。结果:治疗后,两组SLE患者PBMC中IFN-γmRNA表达低于正常,IL-4、IL-10mRNA表达仍高于正常;但处理组上述细胞因子产生水平均降至正常;而对照组IFN-γ产生水平低于正常,IL-4、IL-10产生水平仍高于正常。处理组疾病活动降低的程度明显优于对照组。结论:卡介菌多糖核酸可能在转录后翻译或翻译后调控机制中某一环节对细胞因子蛋白质进行干预,从而对SLE发挥辅助治疗作用。Objective: To investigate the effects of BCG-PSN on the Th cytokine production and its mRNA expression on PBMCs from patients with SLE and study its mechanism. Methods: Sixty-six cases of SLE were randomly divided into two groups; 34 cases in the treatment group ( BCG-PSN plus corticosteroids )and 32 cases in the control group ( corticosteroids alone). The levels of IFN-γ?IL-4 and IL-10 mRNA expression and production were respectively determined by RT-PCR and ELISA before and after treatment. Results: After treatment; the IFN-γmRNA expression of the two groups was lower than those of the normal; while IL-4;IL-10 mRNA expression was still higher than that of the normal. In the treatment group; all the cytokine production decreased to normal. In the control group; IFN-γ lowered in comparison with the normal control; but IL-4?IL-10 were still higher than those in the normal control. The disease activity in the treatment group was reduced much more than that in the control. Conclusions: BCG-PSN could adjust the levels of Th cytokines after transcription and help to relieve the disease activity of SLE. It proves that immunomodulator BCG-PSN could be useful in the treatment of SLE.
