摘  要:目的 探讨抗菌抗炎药物缓释片在肢体爆炸伤中的早期应用及其在延迟伤口外科处理时间中的作用。方法 选用环丙沙星、双氯芬酸钠作为抗菌、抗炎药物 ,甲壳素、卡波姆作为缓释药物的骨架材料 ,直接压片法制得药物缓释片 ,并应用于伤道局部 ,观察伤道组织的抑菌效果 ;利用高效液相色谱法等技术 ,测定药物的溶出度及其在局部组织与血浆中的浓度 ;观察组织病理、伤区细菌数量、伤区组织氧分压及组织压力变化。结果 在 36小时内缓释片向伤道周围组织稳定释放两种药物 ,环丙沙星在伤区组织中达到最低抑菌浓度 ,具有明显的杀菌作用 ;双氯芬酸钠在伤区组织中达到有效浓度。药物缓释片能降低伤区组织压力 ,减少炎细胞侵润及组织继发性损害 ,改善组织活力。结论 抗菌抗炎药物缓释片在平。Objective To investigate the therapeutic effect of sustained release compound tablet containing anti bacterial and anti inflammatory drugs on explosive wounds. Metheds The tablet which contains ciprofloxacin and diclofenac sodium was made by direct mechanical compression,in which Chitin and Carbomer as framework material were used for constant release of the drugs. The tablet was put into the wound tract. The release properties of the sustained release tablet were measured. The changes of the histopathology,the pressure of oxygen,the pressure in tissue,and bacterial number were measured and monitored. Results The tablet could release constantly the drugs for 36 hours and the concentration of the drugs in tissue reached the requirement. Diclofenac sodium can relieve hyper inflammatory response and secondary injuries. Ciprofloxacin can delay the occurence of infection. Conclusion The new tablet has great practical value in early treatment of explosive wounds.
