摘 要:目的 观察针刺治疗Graves'眼病(Graves' ophthalmopathy,GO)患者突眼症状疗效,以及患者血清对抗体依赖细胞介导的细胞毒(ADCC)作用的影响。方法 将54例GO患者随机分为三组,其中针药Ⅰ组18例,针药Ⅱ组18例,对照组18例,观察不同穴位组方结合他巴唑对突眼度的影响,并采用乳酸脱氢酶(LDH)释放试验,观察治疗前后患者血清对培养的人眼外肌细胞的ADCC作用。结果 针药Ⅱ组治疗突眼有效率高于针药I组(P<0.05),对照组与针药治疗组比较差异均有显著性意义(P<0.01)。治疗后GO患者血清对眼外肌的ADCC作用较针刺前明显降低,且针药Ⅱ组低于对照组(P<0.05)。结论 针刺对GO眼病患者突眼的治疗作用可能与减轻血清对眼外肌的ADCC损伤过程有关;针刺远近配穴对抗眼肌损伤的免疫调节作用优于单纯局部取穴。Objective To observe exophthalmia symptom changes after acupuncture treatment in 54 cases of Graves' ophthalmopathy and the influence of the patients' serum to antibody dependent cell-mediated cytotoxicity (ADCC). Methods 54 cases of patients were divided into 3 groups in random, including 18 cases of group Ⅰ, 18 cases of group Ⅱ and 18 cases of control group. To study the influence on the degree of exophthalmia by acupuncturing different points combined with Tapazole. ADCC of the patients' serum to cultured human eye muscle cells were measured by LDH releasing test. Results The effective rate to exophthalmia of group Ⅱ was higher than group Ⅰ(P<0.05). There was significant difference between control group and acupuncture combined drug groups (P<0.01). After treatment, ADCC of the patients' serum to cultured human eye muscle cells was markedly decreased. Group Ⅱ decreased more than control group (P<0.05) . Conclusion The results suggested that the therapeutic effect of acupuncture to exophthalmia may be related with thedecreasing effect of serum to ADCC injury process in eye muscle. The immunoregulating effect of using local and removal points is better than using local points only.
