大隐静脉移植在开放性四肢血管损伤的应用

摘　　要：目的　探讨大隐静脉移植修复开放性四肢血管损伤的作用。方法　对 1 3例采用大隐静脉移植治疗的开放性四肢血管损伤的病例进行分析 ,并阐述大隐静脉的解剖特点、切取方法、血管吻合方法。结果　移植大隐静脉长度 1 3～ 4 5 cm,一个肢体修复血管 3～ 8条 ,伴有肢体浅静脉损伤 4例 ,修复 3例。根据吻合的血管口径不同 ,分别采用“六种”血管吻合方法。移植的血管通畅率 1 0 0 %。结论　大隐静脉能满足开放性四肢血管损伤对移植血管的要求 。Objective To explore the advantages of great saphenons vein(GSV) transplantation in treament of svere open extremity vascular injuries.Methods Thirteen cases adopted the treatment of GSV transplentation for severe open extremity vascular injuries were analyzed.The anatomy characteristics,anatomic methods and vascular anastomosis methods of GSV were expounded.Results The length of GSV to be transplanted was 13～45 cm.The number of the vessels repaired(each extrmity)was 3 to 8.And at the same time 3 cases of limb shallow veins were repaired in 4 cases.Six different ways of vascular anastomosis ware adopted according to vascular diameters.The survival rate was 100%.Conclusion The GSV may fully meet the needs for transplanted vessels of the severe open extremity vascular injuries.Attension should be paid to the repairment of the shallow veins of the extremity when blood vessles are transplanted.
