静吸全麻在妇科腹腔镜手术中的应用

摘　　要：目的　观察静吸复合全麻在妇科腹腔镜手术的应用。方法　 ASA ～ 级妇科患者 4 0例 ,于腹腔镜下行妇科手术 ,采用咪唑安定 -维库溴胺 -乙托咪酯 -芬太尼快速静脉诱导气管插管 ,接麻醉机控制呼吸 ,术始给予芬太尼 ,术中吸入安 (异 )氟醚维持 ,酌情追加维库溴胺。连续监测心电图 (EKG)、平均动脉压 (MAP)、心率 (HR)、脉搏氧饱和度 (Sp O2 ) ,并记录气腹前、气腹后 5 min、1 5 min、放气后 5 min的各参数值。结果　本组患者麻醉效果满意 ,全组患者术毕 1 0 min内完全清醒 ,无气腹和麻醉后相关并发症 ,术中 MAP,HR,Sp O2 无明显波动 (P>0 .0 5 )。结论　静吸复合全麻用于妇科腹腔镜手术 ,血流动力学平稳 ,术毕清醒快 。Objective To observe the application of combined analgesia in gynecological peritoneoscope.Methods Fourty pantients(ASA class Ⅰ～Ⅱ)underwent gyncological operations with peritonescope and received rapid induction intubation with midazolam etomidate fentanyl vecuronium.At the beginging of the operation,we gave fentanyl,maintained anesthesia with enflurane(isoflurane),gave vecuronium when necessary during the operation.After pneumoperitonium,the patients were in Trendelenburg position.We monitored EKG,MAP,HR,SpO 2 continuosly and recorded them before pneumoperitonium,after pneumoperitonium at 5 and 15 min,5 min after peritoneal defflation.Results Anesthetic effect in all patients were satisfactory,all patients recovered in 10 min after the operations.There were no complications correlated with pneumoperitonium and anesthesia.There were no significant differences during operations of MAP,HR,SpO 2( P >0.05).Conclusion The usage of combined analgesia in gynecologieal peritoneoscope has steady hemodynamic effects,rapid recovery,so it is safe and reliable.
