摘  要:目的 探讨细胞间黏附分子 - 1(ICAM - 1)及其调节因子白细胞介素 - 1β在脊髓缺血 -再灌注损伤中的表达和作用。 方法 建立大鼠急性脊髓缺血 -再灌注损伤模型 ,采用逆转录 -聚合酶链反应、免疫组化及免疫荧光激光共聚焦扫描显微镜技术 ,检测脊髓再灌注损伤血管内皮ICAM - 1mRNA和白细胞介素 - 1β(IL - 1β)mRNA表达量。 结果 正常对照组和单纯缺血组不引起细胞因子和黏附分子表达量的增加 (P >0 .0 5 ) ,而再灌注后缺血区细胞因子、黏附分子的表达及多形核白细胞 (PMN)浸润先后发生了改变。缺血 30min后再灌注 2h ,IL - 1βmRNA的表达量为 (1.0 76± 0 .330 )V ,约为正常对照组的 2倍 (P <0 .0 1) ;再灌注 6h达到高峰 ,并持续至 12h。I CAM - 1mRNA表达量于再灌注 4h为 (0 .94± 0 .12 )V (P <0 .0 1) ;再灌注 12h ,其单位微血管面积内ICAM - 1蛋白荧光强度约比单纯缺血组增加了 3倍 (P <0 .0 0 1)。 结论 脊髓缺血 -再灌注损伤时 ,ICAM - 1及其调节因子IL - 1β在继发性脊髓损伤过程中起重要作用。Objective To investigate effects and expressions of intercellular adhesion molecule-1 (ICAM-1) and its regulating factor interleukin-1β (IL-1β) in the spinal cord ischemia-reperfusion injury. Methods The model of spinal cord ischemia-reperfusion was set up to check the expressions of ICAM-1mRNA and IL-1βmRNA of vascular endothelium in the spinal cord reperfusion injury by means of RT-PCR,immunohistochemistry,immunofluorescence and laser confocal scanning microscopy. Results The expressions of cytokines and adhesion molecules showed no increase in the normal group and the ischemia group ( P >0.05). While the expressions of cytokines and adhesion molecules in the ischemic area and the infiltration of neutrophilic granulocyte changed successively after reperfusion. The amount of the expression of IL-1 mRNA increased to (1.08±0.33) V in the spinal ischemia 30 min after two-hour reperfusion,almost twice as much as that of the control group ( P <0.05),and reached the peak lasting for 12 hours after six-hour reperfusion. The amount of the expression of ICAM-1mRNA reached (0.94±0.12) V after four-hour reperfusion ( P <0.01). After 12-hour reperfusion,the expression fluorescent intensity on per microvascular area increased by three times more than that of the ischemia group ( P < 0.01) . Conclusion After the ischemia-reperfusion injury,ICAM-1 and IL-1β play important roles in the secondary spinal cord injury.
