摘  要:目的:观察理肠中药方对溃疡性结肠炎大鼠结肠细胞凋亡及其调控基因Bcl-2,Bax,Fas蛋白表达的影响,分析其作用机制. 方法:应用2,4-二硝基氯苯(DNCB)免疫加醋酸局部灌肠法建立UC大鼠模型,将98只健康SD大鼠(雌雄各半), 按雌雄随机分7组,分别为乌梅丸组、白头翁汤组、参苓白术散组、痛泻要方组、SASP组、模型组和正常组, 分别观察治疗后大鼠结肠黏膜的结肠细胞凋亡及其调控基因Bcl-2,Bax,Fas蛋白表达的变化. 结果:模型组与正常组比较AI升高(P<0.01,t=3.835),治疗后AI下降,各组与模型组比较有显著性差异(P<0.05, q=4.210 vs 痛泻要方组或P<0.01,q=5.973 vs 乌梅丸组,q=5.986 vs 白头翁汤组,q=5.905 vs 参苓白术散组,q=5.889 vs SASP组).与正常组比较均无显著性差异(P>0.05,q=3.972 vs 乌梅丸组,q=3.523 vs 白头翁汤组,q=3.694 vs 参苓白术散组,q=3.549 vs 痛泻要方组,q=3.727 vs SASP组),理肠中药方、SASP组五组间比较,均无显著性差异(P>0.05,F=2.96),正常组Bcl- 2/Bax>1,细胞较少发生凋亡,而二者比较,有显著意义(P<0.01,t=3.956 vs 模型组).理肠中药方、SASP组五组Bcl-2/Bax>1,模型组Bcl-2,Bax,Fas与正常组比较, 有非常显著意义(P<0.01,t=4.573,4.826,4.758),且模型组Bcl-2/Bax<1细胞凋亡占优势,各治疗组与正常组无统计学意义(P>0.05,F=2.69),而与模型组比较有显著意义(P<0.05,q=4.98 vs乌梅丸组,q=4.77 vs白头翁汤组,q=4.68 vs参苓白术散组,q=4.56 vs痛泻要方组,q=4.64 vs SASP组),各中药组Bcl-2,Bax,Fas蛋白阳性率均与SASP组疗效相当(P>0.05,F=3.19,3.05, 2.97),理肠四方各组比较,Bcl-2,Bax,Fas蛋白阳性率亦无明显差异(P>0.05). 结论:细胞凋亡及其调控基因Bcl-2,Bax参与UC的形成,中药及SASP均有良好的治疗效果.AIM: To study the immunoregulatory and treatment effects of Lichangsifang (LCSF) on apoptosis and protein expression of Bcl-2, Bax and Fas genes in rats with ulcerative colitis (UC), and to analyze its underlying mechanism. METHODS: In the experiment, ninty-eight SD rats were randomly divided into seven groups: normal control group, pathologic control group, solfasalazine (SASP) group, Wumeiwan (WMW) group, Baitouwengtang (BTWT) group, Senglingbaishusan (SLSS) group, and Tongxieyaofang (TXYF) group. Every group had fourteen rats (seven male and seven female). Except the normal control group, A UC animal model was made with DNCB and acetic acid the other six groups, which were treated by distilled water, SASP, TXYF, WMW, BTWT, SLBSS and TXYF, respectively. After these treatment, the changes of apoptosis and protein expression of Bcl-2, Bax and Fas gene in UC were observed. RESULTS: The level of apoptotic index (AI) in pathologic group was significantly higher than that in normal group (P<0.01 t=3.835). After these treatment, the degree of the AI decreased in each experimental group, as compared with the pathologic group (P<0.05, q=4.210 vs TXYF group or P<0.01, q=5.973 vs WMW group, q=5.986 vs BTWT group, q=5.905 ivs SASP group, q=5.889 vs SASP group). The apoptosis of normal control group (Bcl-2/Bax>1) togther with LCSF and SASP groups was less (P<0.01 q=3.972 vs WMW group, q=3.523 vs BTWT group, q=3.694 vs SLBSS group, q=3.549 vs TXYF group, q=3.727 vs SASP group). There was remarkable difference in the degree of Bcl-2 and Bax expression between pathologic group (Bcl-2/Bax<1) and normal group (P<0.01, F= 2.96). The degree of apoptosis in pathologic group was significantly higher than that in normal group (P<0.05, t=3.956 vs pathologic group). There was no remarkable difference between LCSF and SASP groups in the protein expression on Bcl-2, Bax and Fas genes in UC (P>0.05, F=3.19, 3.05, and 2.97).
