性激素替代疗法在治疗卵巢早衰中的应用

摘　　要：目的　探讨性激素替代疗法 (HRT)在治疗卵巢早衰 (prematureovarianfailure ,POF)中的作用以及POF的促排卵方案。方法　对 35例因不孕症来本中心的POF患者采用前瞻性研究 ,并于HRT 3～ 6个周期后 ,监测用药前后临床症状、子宫卵巢超声、血清生殖激素水平及促排卵后卵泡发育、受孕情况。结果　 35例患者经HRT后 ,9例临床症状改善 ,血清卵泡刺激素 (FSH)与黄体生成素 (LH)水平明显下降 (P均 <0 .0 5 )。血清雌激素(E2 )水平明显上升 (P <0 .0 5 ) ,停药后 1例自然受孕。 1例行IUI受孕。给GnRHa HMG HCG后 9例有排卵 ,2例行IVF ET方案受孕。结论　性激素替代疗法在治疗卵巢早衰的过程中 ,可以改善其临床症状 ,血清LH、FSH明显下降 ,E2 水平显著升高 。Objective To evaluate hormone replacement treatment (HRT) in patients with premature ovarian failure (POF) and explore the effective inducing ovulation protocol.Methods HRT was used in 35 POF patients and after 3～6 cycles the changes of symptoms,the uterus and ovarian ultrasonography shows,serum hormone level pre-and post-HRT were observed.The growth of oocytes and pregnancy rates were also observed.Results After HRT,clinical symptoms in 9 patients were improved.Serum FSH and LH level decreased significantly (P<0.05) and serum E 2 increased significantly (P<0.05) compared with those before HRT.After stopping HRT,one spontaneously got pregnant.One patient got pregnant after the method of IUI.Nine ovulated after GnRHa/HMG/HCG.Two patients got pregnant after the method of IVF-ET.Conclusion HRT can improve symptoms and decrease the level of serum FSH and LH and also can increase the level of serum E 2 significantly,which are helpful to the ovulation induction.
