摘 要:目的探讨严重腹腔感染状态下大鼠胃黏膜血流量和Na+ K+ ATP酶活性变化对胃黏膜电位差的影响。方法利用大鼠盲肠结扎穿孔造成腹腔严重感染动物模型 ,应用激光多普勒微循环血流计和电生理记录仪检测穿孔前和穿孔后 3、6、12、2 4、4 8h胃黏膜血流量和胃黏膜电位差变化 ;应用生化法测定各时相大鼠胃黏膜Na+ K+ ATP酶活性。结果胃黏膜血流量 (mV)在盲肠穿孔后 3h (43 7± 2 8)与对照组 (5 7 9± 2 7)相比明显降低 (P <0 0 5 ) ,12h(31 9± 2 6 )降至最低(P <0 0 1) ,2 4h(44 7± 2 7)开始回升 ,4 8h(5 2 9± 2 8)仍低于对照组。Na+ K+ ATP酶活性(nmolpi/min/mg蛋白 )在盲肠穿孔后 3h(113± 14 )较对照组 (15 3± 12 )显著降低 (P <0 0 5 ) ,12h(92± 10 )降至最低 (P <0 0 1) ,仅为对照组 (15 3± 12 4 )的 4 8 5 % ,4 8h(12 8± 13)仍未恢复正常。胃黏膜电位差 (mV)在盲肠穿孔后 6h(15 2± 1 4 )较对照组 (2 3 1± 1 6 )显著下降 (P <0 0 5 ) ,12h(11 6± 1 4 )降至最低 (P <0 0 1) ,2 4h(17 0± 1 5 )仍显著低于对照组 [(2 2 6± 1 6 ) ,P <0 0 5 ],4 8h(18 9± 1 5 )显著低于对照组 [(2 2 4± 1 5 ) ,P <0 0 5 ]。结论严重腹腔感染状态下胃黏?Objective To investigate the effect of gastric mucosal blood flow and Na + -K + -ATPase activity on gastric transmucosal potential difference during severe intraperitoneal infection in rats. Methods A rat model of intraperitoneal infection was established by cecal ligation and puncture(CLP). Laser Doppler blood flowmeter was used to examine the gastric mucosal blood flow and electric-physiological recorder was applied for measuring gastric mucosal potential difference. Assay of Na + -K + -ATPase activity in gastric mucosal tissue was conducted by biochemistry method. Results Gastric mucosal blood flow(GMBF,mV) in infected group was significantly lower than in control group(43.7±2.8 vs. 57.9±2.7,P <0.05) at 3 hours after CLP,and rapidly dropped to the lowest(31.9±2.6,P <0.01) at 12 hours post-perforation. By the time of 48h (52.9±2.8) following CLP,GMBF remained lower in infection group than that in control group. The activity of Na + -K + -ATPase(nmol pi/min/mg proten) decreased in infected group at 3 h(113±12 vs. 153±14,P <0.05). There was a minimum of Na + -K + -ATPase activity at 12 h in infected group ( 92± 10 vs. 153±124,P <0.01).Gastric transmucosal potential difference(GTPD,mV)decreased compared with the control group (15.2±1.4 vs. 23.1±1.6,P <0.05) at 6 h,and rapidly dropped to the lowest at 12 h (11.6±1.4 vs. 23.5±1.5,P <0.01). Conclusion Decreased GMBF and Na + -K + - ATPase activity may play an important role in gastric mucosal barrier damage following severe abdominal infection induced by CLP.
