多普勒超声对高血压患者右室舒张功能的临床研究

摘　　要：目的　采用多普勒超声心动图检测三尖瓣口血流有关参数 ,研究原发性高血压 (EH)患者的右室舒张功能及其相关因素。方法　 4 3例 EH患者 ,32例正常人 ,取心尖四腔观 ,将脉冲多普勒 (PWD)取样容积置于三尖瓣口 ,记录其血流频谱 ,测量舒张早期 E峰流速 (E)、舒张晚期 A峰流速 (E)和 E/ A比值 ,舒张早期 E波流速积分 (EVTI) ,舒张晚期 A波流速积分 (AVTI)及 EVTI/ AVTI比值。结果　 1 .与对照组比较 ,EH组 E波和 EVTI下降 (P<0 .0 5 ) ,A波和 AVTI增加 (P<0 .0 1 ) ,E/ A与 EVTI/ AVTI比值降低 (P<0 .0 1 ) ;2 .EH患者三尖瓣 E/ A比值 (r=0 .4 1 7,P=0 .0 0 5 )、 EVTI/ AVTI比值 (r=0 .5 5 7,P=0 .0 0 0 1 )与二尖瓣相应指标显著相关 ;3.EH患者各级血压间这些指标比较无差异 (P>0 .0 5 )。结论　 EH患者的右室舒张功能明显下降 ,与左室舒张功能密切相关 。Objective To study diastolic function of the right ventricle and related factors in essential hypertension (EH) patients by measuring hemodynamic parameters at the tricuspid valve using Doppler echocardiography.Methods Data were obtained from 43 EH patients and 32 normal people (control) On the image from the apex of the heart with the four chambers,We put pulse wave Doppler within the area around the tricuspid valve and recorded the spectrum of tricuspid blood flow Values were measured including diastolic early peak velocity(E),diastolic late peak velocity (atria systolic peak velocity,A) and the ratio of E/A,diastolic E wave velocity integral (EVTI),diastolic A wave velocity integral(AVTI) and the ratio of EVTI/AVTI were measured.Results 1 Compared to the control group,the E and EVTI decreased( P <0 05),the A and AVTI increased in the EH group ( P <0 01),and E/A as well as EVTI/AVTI ratios in the EH group also decreased( P <0 01);2 The ratio of E/A( r =0 417, P =0 005) and the ratio of EVTI/AVTI( r =0 557, P =0 0001) of EH patients' tricupid valve were associated to the corresponding mitral valve indices 3 Right ventricular diastolic function indices had no differences between all degrees of EH patients blood pressure( P >0 05).Conclusions The diastolic function of the right ventricle in EH patients are apparently impaired It is closely related to the left ventricular diastolic function,but does not correlate with degrees of hypertension.
