摘  要:目的 采用高分辨率超声技术评估冠心病患者肱动脉内皮功能受损情况。方法 通过选择性冠状动脉造影确定冠心病组患者 2 8例 ,非冠心病组 2 6例 ,在冠状动脉造影前后 2 4 h内进行肱动脉超声检查 ,比较反应性充血和含服硝酸甘油后肱动脉内径和血流变化 ,并对冠心病易患因素做比较。结果 冠心病组较非冠心病组肱动脉血流介导的血管扩张反应 ( FMD)显著减低 ( 4.1 7%± 2 .98% ) vs( 1 0 .2 6 %± 8.4 1 % ) ,P=0 .0 3,但两组含服硝酸甘油后血管内径无明显差异。两组的冠心病易患因子数 ( P=0 .0 1 )和吸烟 ( P=0 .0 0 4 )有显著性差异。结论 应用高分辨率超声技术可非侵入性评价周围血管内皮功能 ,冠心病患者通常存在肱动脉 FMD受损。Objective To evaluate the impairment of the flow mediated vasodilation(FMD) of the brachial artery in patients with coronary artery disease(CAD) using B mode high resolution ultrasound. Methods Angiography revealed CAD(≥50% diameter stenosis) in 28 patients (CAD group) and normal coronary arteries in 26 patients(non CAD group) Vasodilation following reactive hyperemia and after sublingual nitroglycerin was assessed in the brachial artery using B mode high resolution ultrasound The different risk factors between the two groups were also compared. Results CAD patients showed markedly impaired FMD compared to the non CAD group (4 17±2 98%,10 26±8 41%, P =0 03) Nitroglycerin induced similar degree of vasodilation in the two groups There were siginificant differentces in the kinds of risk factors and cigarettete smoking between the two groups. Conclusions This study indicates that the impairment of FMD in the brachial artery,a marker of systemic endothelial function,is closely related to CAD The non invasive assessment of flow mediated dilation in the brachial artery can be used as a kind of methods to measure coronary endothelial function
